摘要(Abstract):
目的:探讨BS330与AR350两种吸附柱对高胆红素血症重症患者的治疗效果。方法:收集同期收治的使用吸附柱治疗的高胆红素血症患者44例,根据患者使用吸附柱情况分为BS330组及AR350组,收集2组患者年龄、性别、APACHE Ⅱ评分、住ICU时间及预后资料;于治疗前及吸附治疗后30 min采集患者全血,检测总胆红素(TBL)、直接胆红素(DBL)、间接胆红素(IDBL)、谷丙转氨酶(AST)、谷草转氨酶(ALT)、胆汁酸(TBA)及血小板(PLT),比较上述各项指标的下降率。结果:2组患者男女构成比、年龄、APACHE Ⅱ评分、住ICU时间及预后比较,差异无统计学意义(P> 0.05);治疗前,2组患者TBL、DBL、IDBL、AST、ALT、TBA及PLT比较,差异无统计学意义(P> 0.05);治疗后,除AR350组IDBL较治疗前明显降低(P <0.05),其余指标比较差异无统计学意义(P> 0.05); 2组患者治疗后TBL、DBL、IDBL、ALT、AST及PLT下降率比较,差异无统计学意义(P> 0.05),BS330组患者TBA下降率明显高于AR350组,差异有统计学意义(P <0.05)。结论:BS330与AR350对高胆红素血症重症患者治疗效果相当,但BS330型降低胆汁酸作用更强。
Objective: To investigate the therapeutic effect of BS330 and AR350 adsorption columns on patients with severe hyperbilirubinemia. Methods: 44 patients with hyperbilirubinemia treated with adsorption columns were collected and divided into BS330 group and AR350 group according to the use of adsorption column. Age,gender,APACHE Ⅱ scores,ICU stay time and prognosis of two groups were collected. TBL,DBL,IDBL,AST,ALT,TBA and PLT were detected before treatment and 30 minutes after adsorption treatment,and the decline rates of the above indicators were compared.Results: There was no significant difference in gender ratio,age,APACHE Ⅱ scores,ICU stay time and prognosis between the two groups( P > 0. 05); Before treatment,there was no significant difference in TBL,DBL,IDBL,AST,ALT,TBA and PLT between two groups( P > 0. 05). After treatment the IDBL of AR350 group was significantly lower than that before treatment( P < 0. 05),while there was no significant difference in other indexes( P > 0. 05),and there was also no significant difference in TBL,DBL,IDBL,ALT,AST and PLT between the two groups( P > 0. 05). TBA decreased rate in BS330 group was significantly higher than that in AR350 group( P < 0. 05).Conclusion: BS330 and AR350 have equivalent efficacy in reducing bilirubin level of patients with severe hyperbilirubinemia,but BS330 has better effect in reducing TBA.
