肺康复联合训练对颈胸段脊髓损伤患者的临床效果Clinical effect of lung rehabilitation combined training in patients with cervicothoracic spinal cord injury

摘要(Abstract):
目的探讨肺康复联合训练对颈胸段脊髓损伤(SCI)患者的临床效果。方法选取颈胸段SCI患者60例,随机均分为对照组(常规康复训练,5 h/d、5 d/周)和观察组(常规康复训练联合肺康复训练,2次/d、5 d/周),连续治疗4周,采用Powerbreathe肺功能测试仪检测2组患者治疗前后的肺呼气功能[第1秒用力呼气容积(FEV1)、用力肺活量(FVC)及FEV1/FVC]和肺吸气功能(吸气肌力指数和吸气峰流速),采用Megwin6000表面肌电测试仪检测2组患者治疗前后的肌电振幅均方根值(RMS),采用Borg量表、改良Barthel指数评分及世界卫生组织生存质量测定量表简表(WHOQOL-BREF)分别评价2组患者治疗前后呼吸困难程度、日常生活活动能力(ADL)及生存质量,同时收集2组患者治疗前后肺部感染发生情况。结果经肺康复联合训练后观察组较对照组患者的肺功能、Borg呼吸困难程度、呼吸肌肌力、ADL、生存质量及肺部感染情况均有改善(P<0.05)。结论肺康复联合训练能够明显改善颈胸段SCI患者肺功能,提高ADL和生存质量,降低肺部感染发生。
Objective To explore the clinical effect of lung rehabilitation combined training in patients with cervicothoracic spinal cord injury(SCI).Methods A total of 60 patients with SCI in the neck and chest segment were selected and randomly divided into control group(routine rehabilitation training,5 h/d,5 d/week) and observation group(routine rehabilitation training combined with pulmonary rehabilitation training,2 times/d,5 d/week) for 4 weeks of consecutive treatment.The Powerbreathe lung function tester was used to detect forced expiratory volume(FEV_1)at the first second,forced expiratory volume(FVC),and FEV1/FVC and pulmonary inspiratory function(flow rate of inspiratory muscle force index and inspiratory peak) before and after the treatment of group 2 patients.Root EMG values before and after treatment in the 2 groups were measured using a Megwin6000 surface EMT.The Borg scale,activity of daily life(ADL) and quality of survival were evaluated by the modified Barthel index score,and summary of the WHO Quality of Survival Scale(WHOQOL-BREF),and the occurrence of lung infection before and after treatment in the two groups.Results After the joint training of lung rehabilitation,the observation group had improved lung function,Borg respiratory difficulty,respiratory muscle muscle strength,ADL,quality of survival,and lung infection in the control patients(P <0.05).Conclusion Combined training of pulmonary rehabilitation can significantly improve lung function,improve ADL and quality of survival,and reduce the incidence of pulmonary infection.
