不同剂量甲状旁腺素对下颌骨牵张成骨新骨质量的影响The Influence of Different Doses of Parathyroid Hormone on Differences of New Bone Formation During Mandibular Distraction Osteogenesis

摘要(Abstract):
目的:研究不同剂量重组人甲状旁腺素(rh PTH)对下颌骨牵张成骨新骨形成及质量的影响。方法:日本大耳白兔45只,随机均分为对照组和实验A、B、C、D组,在前牙和第一磨牙交界处垂直于下颌骨下缘,用慢速裂钻作截骨线,牵张延长下颌骨10 mm,对照组动物隔日皮下注射生理盐水1 m L,实验A、B、C及D组分别隔日皮下注射rh PTH 10、20、30、40μg/kg,在固定期第1、7及14天时行牵张区新骨组织形态学研究和骨密度检测。结果:各固定期实验组的成骨细胞数量、骨小梁面积和骨密度均较高,与对照组比较差异有统计学意义(P<0.05);在固定期第1天时实验C组的平均成骨细胞数量和骨密度最高,随固定期延长,各组新骨骨密度和骨小梁面积逐渐升高,而成骨细胞数量逐渐减少。结论:间断皮下注射rh PTH可以促进下颌骨牵张成骨新骨生成,不同剂量rh PTH促进下颌骨牵张成骨可能存在剂量效应差异。
Objective: To investigate the effects of parathyroid hormone(PTH) on new bone formation during mandibular distraction osteogenesis( DO). Methods: Forty-five Japnese rabbits were divided into control group,receiving a subcutaneous injection of 1 ml of saline every other day and experimental groups A,B,C and D,receiving subcutaneous injections of 10,20,30 and 40 μg / kg of rh PTH,respectively,every other day. On the 1st,7th,14 thday of the consolidation period after the mandibular distraction had been completed,new bone in the distraction region was examined through histomorphometric investigation and bone mineral density( BMD) was detected. Results: On the 1st,7th,14 thday of the fixation period,number of osteoblasts,trabecular bone area and BMD were significantly greater in each experimental group than their counterparts in control group( P < 0. 05). On 1st day of the consolidation period,the average gnumber of osteoblasts and BMD in group C were highest.With the fixation period to extend,in each experimental group,BMD of new bone and trabecular bone area increased gradually while number of osteoblasts decreased. Conclusions: Intermittent subcutaneous injections of rh PTH can promote new bone formation during mandibular DO. Different doses of rh PTH can promote mandibular DO and there may exist differences in dose effect.
