肾素原受体在IgA肾病中的表达及意义Expression of Renin Receptor in IgA Nephropathy

摘要(Abstract):
目的:探讨肾素原受体(PRR)在IgA肾病组织中的表达及意义。方法:收集肾穿刺组织标本74例,其中IgA肾病54例[IgA肾病组,包括局灶增生型20例、IgA肾病合并膜性肾病(MN)20例、14例新月体型]、微小病变肾病(MCD)20例(对照组),采用免疫组织化学法染色、显微镜观察肾组织PRR的表达,并分析PRR相对表达量与IgA、IgG、IgM、C3、C1q及Fib在肾小球沉积的关系。结果:PRR在IgA肾病组和对照组的肾小球和肾小管均有表达,IgA肾病组的表达量高于对照组(P<0.05);在IgA肾病组,随着病变程度的加重,PRR在肾小球区域的表达逐渐增加(P<0.05),在肾小管区域的表达量逐渐减少(P<0.05); PRR在肾小管区表达与IgA、IgM及Fib沉积呈正相关,与IgG的沉积呈负相关,但在肾小球区的表达水平与IgA及Fib的沉积呈负相关(P<0.05)。结论:PRR在IgA肾病患者肾组织中表达水平高于MCD患者,并随着病变加重,PRR表达水平在肾小球呈上升趋势,在肾小管呈下降趋势,提示肾素-血管紧张素系统可能在IgA肾病的发生发展过程中有一定作用。
Objective: To investigate the expression and significance of renin receptor(PRR) in IgA nephropathy. To investigate the expression and significance of prorenin receptor(PRR/ATP6 AP2) in renal tissue of immunoglobulin A(IgA) nephropathy. Methods: 74 cases of renal puncture tissue specimens were collected, including 54 cases of IgA nephropathy(IgA nephropathy group, including 20 cases of focal hyperplasia, 20 cases of membranous kidney disease and 14 cases of crescent type) and 20 cases of minimal lesion nephropathy(control group). The expression of PRR in renal tissue was observed by immunohistochemistry, and the relationship between the relative expression of PRR and the deposition of IgA, IgG, IgM, C3, C1 q and Fib in glomeruli was analyzed. Results: PRR was expressed in glomeruli and tubules of IgA nephropathy group and control group, and the expression of IgA in kidney disease group was higher than that in control group(P<0.05). In IgA nephropathy group, the expression of PRR increased gradually with the severity of lesion(P<0.05), but the expression in the renal tubule decreased gradually(P<0.05). The expression of PRR in renal tubules was positively correlated with IgA, IgM and Fib deposition, and negatively correlated with IgG deposition. The expression level in glomerular area was negatively correlated with the deposition of IgA and Fib(P<0.05). Conclusion: The expression level of PRR in renal tissue of patients with IgA nephropathy is higher than that of patients with MCD. With the aggravation of pathological changes, the expression level of PRR increases in glomeruli and decreases in renal tubules, suggesting that renin-angiotensin system plays a role in the occurrence and development of IgA.
