贵州省少数民族地区老年住院患者营养状况调查
Nutritional Status Inquisition among Elderly Inpatients in Minority Areas of Guizhou China

摘要(Abstract):

目的:了解贵州省部分少数民族地区老年住院患者营养不良、营养风险和营养支持的现状。方法:收集贵州省黔东南苗族侗族自治州,黔西南布依族苗族自治州和黔南布依族苗族自治州三级甲等综合医院的老年住院患者815例,采用微量营养评估-短表(MNA-SF)评估患者营养状况,营养风险筛查量表(NRS2002)评估患者营养风险;比较各地区、地区内男女患者和各科室营养不良和营养风险发生的比率,并分析营养支持的使用情况。结果:贵州省少数民族地区的整体营养不良和营养风险的检出率分别为42.3%和57.1%。其中,3个地区间营养不良的检出率有差异(黔东南地区>黔西南地区>黔南地区),具有统计学意义(P<0.05);而各个科室营养不良检出率波动范围在30.7%~50.8%,其中以神经内科检出率最低,心内科检出率最高;各科室间营养风险检出率的波动范围在34.6%~69.2%,手术科室和非手术科室之间有差异,具有统计学意义(P<0.05),其中以肝胆内科最低,消化内科最高;在815例少数民族地区老年住院患者中,共有225例使用了营养支持,使用率达到了27.61%,而且非手术科室有营养风险的老年住院患者营养支持使用率低于手术科室,差异具有统计学意义(χ2=86.989,P<0.01);非手术科室无营养风险的老年住院患者营养支持使用率同样低于手术科室,差异具有统计学意义(χ2=51.898,P<0.01)。结论:贵州省部分少数民族地区老年住院患者的营养不良和营养风险水平相对较高,存在不合理使用营养支持的情况。
Objective: To investigate malnutrition,nutritional risks and application of nutritional support in elderly inpatients in some minority areas of Guizhou Province China. Methods: 815 elderly inpatients in third level Grade A general hospitals of Qiandongnan Miao-Dong Autonomous Prefecture,Qianxinan Buyi-Miao Autonomous Prefecture and Qiannan Buyi-Miao Autonomous Prefecture in Guizhou China were involved in the study. The Mini Nutritional Assessment short-form( MNA-SF) and Nutritional Risk Screening( NRS 2002) were used to evaluate the malnutrition or nutritional risks for elderly inpatients in different areas,sex and divisions. Results: The overall prevalence of malnutrition and nutritional risks were 42. 3% and 57. 1% restropectively. In addition,the difference of the prevalence of malnutrition in three regions,was statistically significant( P < 0. 05). The fluctuation range of malnutrition rate in various departments was 30. 7% ~ 50. 8%,with the highest positive rate in departments of cardiology; The fluctuation range of nutritional risk prevalence in various departments was34. 6% ~ 69. 2%,with the lowest positive rate in departments of hepatobiliary medicine,and the highest positive rate in departments of gastroenterology( P < 0. 05); The utilization rate of nutritional support was up to 27. 6%,nutritional support in non-surgical elderly inpatients with nutritional risk was lower than that in surgical departments( χ2= 86. 989,P < 0. 01) and the utilization rate of nutritional support with no nutrition risk was the same as the former( χ2= 51. 898,P < 0. 01). Conclusion: The malnutritional rate and nutritional risk rate of elderly inpatients from Guizhou minority areas remained at a high level. In addition,and there exists unreasonable abuse of nutritional support.
