维持性血液透析患者微炎症状态、瘦素与并发心血管疾病的关系
The Relationship of Micro-inflammatory State and Leptin Expression in Maintenance Hemodialysis Patients with Complicating Cardiovascular Diseases

摘要(Abstract):

目的:观察维持性血液透析患者微炎症状态反应指标C反应蛋白、瘦素在合并心血管疾病组和无心血管疾病组之间的关系。方法:以维持性血透合并心血管疾病者22例、维持性血透无心血管疾病者16例、正常对照组13例为研究对象,ELISA法检测瘦素水平,免疫比浊法检测C反应蛋白水平。结果:心血管疾病血透组和未合并心血管疾病血透组瘦素、C反应蛋白水平均明显高于正常对照组(P<0.05,P<0.01),且心血管疾病组瘦素、C反应蛋白水平显著高于无心血管疾病组(P<0.05);在维持性血透患者中瘦素与C反应蛋白呈正相关关系(r=0.362,P<0.05)。结论:瘦素可能参与维持性血透患者心血管疾病的发病,可作为血透患者心血管疾病发生的预警指标;维持性血透患者的微炎症状态可能是瘦素水平升高的直接或间接原因。
Objective: To investigate the micro-inflammatory state and the expression of peripheral blood leptin in maintenance hemodialysis patients and their relationship with the occurrence of cardiovascular diseases.Methods: Twenty-two maintenance hemodialysis(HD) patients with cardiovascular disease(group HD1),16 maintenance hemodialysis patients without cardiovascular disease(group HD2) and 13 healthy people(control group) were enrolled in this study.Leptin was detected by ELISA.C-reaction protein(CRP) was detected with Immunology methods.Statistic analysis was performed on the data with SPSS software.Results: Serum leptin and CRP levels of maintenance hemodialysis patients were significantly higher than those of controls(P<0.05,P<0.01),and serum leptin and CRP levels of patients in group HD1 were significantly higher than those of patients in group HD2(P<0.05).Positive correlation was observed between leptin and CRP in maintenance hemodialysis group(r=0.362,P<0.05).Conclusions: Microinflammatory state and leptin might be involved in the pathogenesis of cardiac disease in maintenance hemodialysis patients,and the level of leptin can be used as an indicator of cardiac disease when hemodialysis.Microinflammation state might be the indirect or direct cause of high leptin expression.
