药敏实验指导治疗婴幼儿川崎病合并肺炎支原体感染
Effect of Drug Sensitivity Test in Guiding the Treatment of Children with Kawasaki Disease Complicated with Peumoniae Mycoplasma Infection

摘要(Abstract):

目的:观察药敏实验指导下婴幼儿川崎病合并肺炎支原体感染的治疗效果。方法:38例川崎病合并肺炎支原体感染婴幼儿,采集患儿咽喉分泌物进行药敏实验,患儿根据药敏实验结果给予抗感染治疗2周;记录患儿治疗前和治疗2周后血液的白细胞(WBC)数、红细胞沉降率(ESR)、超敏C反应蛋白(CRP)、血红蛋白含量(Hb)及血小板(PLT)数;观察药敏实验指导婴幼儿川崎病合并肺炎支原体感染的治疗效果。结果:药敏结果显示,阿奇霉素的敏感性最高(97.37%),其次为利福霉素(89.47%),耐药性最高的是克拉霉素(23.68%)、依托红霉素(18.42%)及克林霉素(18.42%);治疗前患儿的WBC数、ESR、CRP、PLT数均高于正常参考值,治疗后患儿WBC计数恢复正常,ESR、CRP及PLT数虽低于治疗前但未恢复到正常参考值;Hb治疗前后均未出现变化;治疗2周后,治愈患儿26例(68.4%),好转11例(28.9%),总有效率为97.3%。结论:药敏实验指导婴幼儿川崎病合并肺炎支原体感染的治疗效果良好。
Objective: To observe the effect of drug sensitivity test in guiding the treatment of children with Kawasaki disease complicated with pneumoniae mycoplasma infection. Methods: Thirtyeight children with Kawasaki disease complicated with pneumoniae mycoplasma infection were selected. Throat secretions of the children were collected for drug sensitivity test,and according to the test results,corresponding anti-infective therapy were carried out for two weeks. WBC,ESR CRP,Hb and PLT were detected before treatment and two weeks after treatment. Curative effects were observed. Results: Drug sensitivity results showed that azithromycin had the highest sensitivity( 97. 4%),followed by rifampicin( 89. 47%),clarithromycin had the highest resistance( 23. 68%),followed by erythromycin( 18. 42%) and clindamycin( 18. 42%). WBC,ESR CRP and PLT before treatment were higher than normal reference value; after treatment,WBC returned to normal,and ESR CRP and PLT were lower than those before treatment but didn't returned to normal. Hb didn't change before and after treatment. 26 cases cured( 68. 4%),11 cases improved( 28. 9%),the total effective rate was 97.3%. Conclusions: The effect of drug sensitivity test in guiding the treatment of children with Kawasaki disease complicated with pneumoniae mycoplasma infection is good.
