贵阳地区育龄人群HbA_2筛查轻型β-地中海贫血截断值的确定

摘要(Abstract):

目的:建立贵阳地区育龄人群血红蛋白A_2(HbA_2)筛查轻型β-地中海贫血的截断值。方法:对152例育龄期男性,129例育龄期女性非孕妇和110例孕妇进行血红蛋白电泳和β-地中海贫血基因检测;以基因检测为金标准,绘制Hb A_2的ROC曲线,确定各组HbA_2筛查轻型β-地中海贫血的截断值。结果:整体的截断值为3.95%,敏感度、特异度和准确度分别为94.8%、99.3%和98.5%;男性组的HbA_2截断值(4.40%)明显高于女性非妊娠组(3.75%)和女性妊娠组(3.70%),女性妊娠状态对HbA_2截断值无明显影响。结论:建立了贵阳地区育龄人群HbA_2筛查轻型β-地贫的截断值,不同性别育龄人群的Hb A_2截断值存在差异。
Objective: To determine the cut-off values of hemoglobin A_2( HbA_2) screening for mildβ-thalassemia in Guiyang population of reproductive age. Methods: Hemoglobin electrophoresis andβ-thalassemia gene tests were carried out in 152 men of reproductive age,129 non-pregnant women of reproductive age,and 110 pregnant women. With genetic testing as the gold standard,receiver operating characteristic( ROC) curves of HbA_2 were drawnto determine the cut-off values of HbA_2 screening for mild β-thalassemia in each group. Results: The overall cut-off valueof HbA_2 was 3. 95%,and sensitivity,specificity,accuracy were 94. 8%,99. 3%,98. 5% respectively. The cut-off value of HbA_2 in male group( 4. 40%) was significantly higher than those of female non-pregnancy group( 3. 75%)and female pregnancy group( 3. 70%). Women in pregnancy status had no obvious influence on cutoff value of HbA_2. Conclusion: The cut-off values of HbA_2 screening for mild β-thalassemia in reproductive population in Guiyang region have been determined. There is certain difference of HbA_2 cut-off value in terms of gender,which has a certain guiding significance inscreening for mild β-thalassemia.
