唇腭裂错(牙合)与骨性Ⅰ类个别正常(牙合)的横向比较
Comparison of transverse in cleft lip and palate malocclusion and classⅠindividual normal occlusion

摘要(Abstract):

目的 探讨唇腭裂错(牙合)患者牙弓与基骨弓的横向关系和特征。方法 选取30例唇腭裂错(牙合)畸形患者作为实验组,30例骨性Ⅰ类个别正常(牙合)患者作为对照组;2组患者均拍摄三维计算机断层扫描(CBCT)测量上下颌基骨弓宽度、比较上下颌基骨弓宽度差,石膏模型上测量牙弓宽度,分析唇腭裂患者横向不调问题。结果 实验组上颌牙弓、基骨弓及下颌牙弓宽度的差异均明显小于对照组(P<0.001),下颌尖牙区基骨弓宽度、上、下颌基骨弓差值明显大于对照组(P<0.001);实验组女性患者牙弓及基骨弓宽度小于男性(P<0.05)。结论 唇腭裂错(牙合)组存在明显上下颌横向不调且有牙齿代偿表现,治疗设计前应予以准确评估。
Objective To explore the malocclusion characteristics and transverse relationship of the dental arch and basal bone arch in patients with cleft lip and palate.Methods Thirty patients with cleft lip and palate deformity served as the experimental group and 30 patients with skeletal classⅠindividual normal occlusion served as the control group. For both groups, take Cone Beam Computed Tomography(CBCT) scan to measure the width of the maxillary, and mandibular basal bone arch and compare the width differences of the maxillary and mandibular basal bone arch. The width of the dental arch was measured on the plaster model. Transverse discrepancies of patients with cleft lip and palate were analyzed.Results The width differences of the maxillary dental arch, base bone arch, and mandibular dental arch in the experimental group were notably smaller than those in the control group(P< 0. 001), and the width of the basal bone arch of the mandibular canine area as well as the width differences of the maxillary and mandibular basal bone arch in the experimental group were significantly larger than those in the control group(P< 0. 001); the widths of the dental arch and basal bone arch of female patients were smaller than those of male patients(P< 0. 05).Conclusion The group of patients with cleft lip and palate has obvious maxillary and mandibular transverse irregularities and dental compensation, which should be accurately evaluated before treatment design.
