放疗前宫颈癌组织IER5表达与中子刀疗效的相关性
The Correlation Between the Expression of IER5 and the Efficacy of Neutron Knife in the Treatment of Cervical Cancer before Radiotherapy

摘要(Abstract):

目的:探讨放疗前宫颈癌组织早期快速反应基因5(IER5)表达与中子刀疗效的相关性。方法:收集放疗前累积接受7. 5 Gy、15 Gy、22. 5 Gy及30 Gy照射剂量治疗的宫颈癌组织标本各25例,分别采用逆转录-聚合酶链式反应(RT-PCR)及免疫组化法检测IER5 mRNA及蛋白表达水平,比较4个照射剂量组高IER5蛋白表达及低IER5蛋白表达宫颈癌患者的临床获益率,Spearman法分析放疗前IER5蛋白表达与中子刀疗效的相关性。结果:不同照射剂量治疗的宫颈癌组织IER5 mRNA及IER5蛋白相对表达量比较,差异有统计学意义(P <0. 05),照射剂量越高,IER5 mRNA和IER5蛋白的相对表达量也越高,两两比较差异有统计学意义(P <0. 05);在累积接受7. 5 Gy、15 Gy、22. 5 Gy、30 Gy照射剂量的宫颈癌患者中,高IER5蛋白表达者临床获益率高于低IER5蛋白表达者,差异均有统计学意义(P <0. 05);放疗前IER5蛋白表达与中子刀疗效呈显著正相关(r=0. 546,P <0. 05)。结论:随累积照射剂量增加会增加宫颈癌患者的临床获益率,其机制可能与照射提高宫颈癌组织中IER5mRNA和蛋白表达量有关,放疗前宫颈癌组织中IER5蛋白与中子刀疗效呈显著的正相关。
Objective: To investigate the correlation between the expression of rapid response gene 5(IER5) and the efficacy(CR + PR + SD) of the neutron knife in the treatment of cervical cancer before radiotherapy. Methods: 25 cases with cervical cancer were collected and received radiation with7. 5 Gy,15 Gy,22. 5 Gy and 30 Gy doses respectively,and the reverse transcription polymerase chain reaction(RT-PCR) was used to detect the relative expression of IER5 and mRNA,the the optical density(OD) of IER5 protein was detected by immunohistochemistry. The relative expression levels of IER5,mRNA and IER5 proteins were compared among the cases with different radiotherapy doses.According to the expression of IER5 protein,the patients were divided into the high expression group and low expression group,and the clinical benefit rates(CR + PR + SD) were compared after irradiation with different doses. Results: The relative expression levels of IER5,mRNA and IER5 protein in different irradiation doses were statistically different(P < 0. 05),and the higher the radiation dose,the higher the relative expression of IER5,mRNA and IER5 proteins. The relative expression of IER5,mRNA and IER5 protein in both samples were statistically significant(P < 0. 05). Among the patients who received 7. 5 Gy,15 Gy,22. 5 Gy and 30 Gy irradiation doses,the clinical benefit rates of high IER5 protein expression were 70. 00%,66. 67%,64. 29% and 78. 95% respectively,much higher than those with low IER5 protein expression by 33. 33%,30. 37%,45. 45%,and 50. 00%,with significant differences(P < 0. 05). There was a positive correlation between the expression of IER5 protein and the efficacy of neutron knife before radiotherapy(r = 0. 546,P < 0. 05). Conclusion: The higher the cumulative radiation dose,the higher the relative expression of IER5 mRNA and protein in cervical cancer tissues. The expression of IER5 protein was positively correlated with the efficacy of neutron knife before radiotherapy,playing a guidance role in the selection of clinical radiotherapy protocols and the prediction of prognosis.
