摘要(Abstract)：
目的 :了解慢性阻塞性肺疾病 (COPD)患者脂质代谢特点及其与病情的相关性。方法 :测定 15 1例COPD患者 (慢支炎肺气肿组 6 5例 ,慢性肺心病组 86例 )及 6 2例正常人血清甘油三酯 (TG)、总胆固醇 (TC)、高密度脂蛋白胆固醇 (HDL -ch)、载脂蛋白A1(APOA1)、载脂蛋白B10 0 (APOB10 0 )的水平。结果 :慢支炎肺气肿组HDL -ch降低、APOB10 0升高 ;慢性肺心病组TG、TC、HDL -ch、APOA1均降低 ,APOB10 0升高 ,APOA1/APOB10 0比值降低。与对照组比较 ,差异均有显著性 (P <0 0 1)。慢性肺心病组APOA1水平较慢支炎肺气肿组下降更为明显 (P <0 0 1)。结论 :COPD患者存在脂质代谢异常。在肺心病阶段脂质代谢紊乱较慢支炎肺气肿阶段更为明显
Objective: To understand the correlation between the lipid metabolism and clinical features in patients with COPD(chronic obstrutcive pulmonary disaease).Methods: Serum triglyceride(TG), total cholesterol(TC),high density lipoprotein-cholesterol(HDL-ch),apolipoprotein A1(APO A1) and apolipoprotein B100(APO B100) were measured in 151 patients with COPD and 62 normal persons as control.Results:The mean values of HDL-ch of the patients with chronic bronchitis were lower than those of the controls,but APO B100 was higher.The mean value of APO B100 of the patients with chronic pulmanary heart disease was highe than that of the controls, but TG, TC,HDL-ch,APO A1 and APO A1/APO B100 were lower.There were statisticatly significant differences between them and controls(P<0 01). The mean values of serum APO A1 in patients with chronic pulmonary heart disease were sighificantly lower than that in the chronic bronchitis patients(P<0.01).Conclusions:There was an abnormality of lipid metabolism in the patients with COPD and the lipid metabolic disorder in the patients with chronic pulmonary heart disease was more significant than that in the chronic bronchitis patiants.
