腹直肌外侧入路与髂腹股沟入路治疗髋臼骨折的临床疗效Comparison of Clinical Effect between the Lateral Rectus Abdominis Approach and the Ilioinguinal Approach in the Treatment of Patients with Acetabular Fractures

摘要(Abstract):
目的:比较经腹直肌外侧入路与髂腹股沟入路治疗髋臼骨折患者的临床疗效。方法:选取髋臼骨折患者30例,随机均分为研究组(腹直肌外侧入路)和对照组(髂腹股沟入路),比较2组患者的解剖显露时间、术前Hb、手术时间、术中出血量、术中输红悬液量、术后Hb、骨折复位优良率、术后髋关节功能及并发症发生率的差异。结果:研究组患者解剖显露时间、手术时间、术中出血量及术中输血量均低于对照组,术后Hb明显高于对照组,差异均有统计学意义(P<005或P<001);研究组患者复位优良率和髋关节功能优良率高于对照组,手术并发症发生率明显低于对照组,差异均有统计学意义(P<005或P<0001)。结论:与髂腹股沟入路相比,腹直肌外侧入路治疗髋臼骨折具有手术时间短、术中出血少、骨折复位及术后关节功能恢复满意、并发症少等优点。
Objective: To compare the clinical effect of lateral rectus abdominis approach and ilioinguinal approach on patients with acetabular fractures.Methods: Thirty patients with acetabular fractures were randomly divided into a study group(lateral rectus abdominis approach) and a control group( ilioinguinal approach). The two groups were compared the anatomic exposure time,hemoglobin(Hb) before and after surgery, operation time, blood loss, intraoperative transfusion volumes of RBC suspension, excellent rate of fracture reduction, postoperative hip function and incidence of complications.Results: The anatomical exposure time, operation time, intraoperative blood loss and intraoperative RBC transfusion volume were lower in the study group than those in the control group(P< 0. 05), while the postoperative Hb and the excellent rate were significantly higher than those in control group(P< 0. 01 orP< 0. 05). Moreover, the incidence of surgical complications was significantly lower than that in the control group(P< 0.05 orP< 0. 001).Conclusion: The lateral rectus abdominis approach is superior to ilioinguinal approach in the treatment of patients with acetabular fracture. The lateral rectus muscle approach the iliac inguinal approach.
