非小细胞肺癌患者化疗前后肠道微生态菌群多样性变化及其与预后的相关性
Correlation between diversity of intestinal microflora and chemotherapy prognosis of patients with non-small cell lung cancer

摘要(Abstract):

目的 探讨与非小细胞肺癌(NSCLC)患者化疗前后肠道微生态菌群多样变化预后的相关性。方法 选取NSCLC患者40例为NSCLC组,另选择同时期健康体检者40例为对照组,采集两组受试者的粪便提取菌群DNA,用定量聚合酶链反应(Q-PCR)检测产丁酸菌的表达水平;比较两组肠道产丁酸菌的差异,NSCLC组化疗前后肠道产丁酸菌差异及肠道菌群的丰富度、多样性指数,记录随访1年NSCLC组患者生存情况,采用Pearson相关性分析NSCLC患者化疗前后肠道菌群的丰富度、多样性指数变化与预后的相关性。结果 NSCLC组患者肠道产丁酸菌低于对照组(P<0.05),且NSCLC患者化疗后肠道产丁酸菌低于化疗前(P<0.05);NSCLC患者化疗后肠道菌群丰富度、多样性指数低于化疗前,差异有统计学意义(P<0.05);化疗前后肠道菌群的丰富度和多样性指数变化幅度与预后有明显的相关性(r=0.214、0.302,P<0.05)。结论 肠道菌群多样性变化与NSCLC患者的预后有相关性。
Objective To investigate the correlation between diversity of intestinal microflora and chemotherapy prognosis of patients with non-small cell lung cancer(NSCLC).Methods A total of40 patients with NSCLC as NSCLC group and 40 healthy people with physical examination during the same period were selected as control group. The feces of both groups were collected to extract the DNA of floras for quantitative polymerase chain reaction(Q-PCR) detection. The differences in intestinal butyrate-produced bacteria of both groups and the difference of intestinal butyrate-produced bacteria of NSCLC group before and after chemotherapy, the richness and diversity index of intestinal flora were compared. At 1 year of follow-up for NSCLS group, the survival was recorded, and the correlation between changes of richness and diversity index of intestinal flora before/after chemotherapy and prognosis was analyzed by Pearson correlation analysis.Results The expressions of intestinal butyrate-produced bacteria in NSCLC group were significantly lower than that of control group(P<0. 05), and the expressions of intestinal butyrate-produced bacteria of patients with NSCLC after chemotherapy were significantly lower than those before chemotherapy(P< 0. 05). The richness and diversity index of intestinal flora in patients with NSCLC after chemotherapy were lower than those before chemotherapy, differences were statistically significant(P< 0. 05). Changes of richness and diversity index of intestinal flora before and after chemotherapy were significantly correlated with prognosis(r= 0. 214, 0. 302;P< 0. 05).Conclusion Changes of diversity index of intestinal flora are correlated with prognosis.
