慢性心衰患者血清SIRT1的表达及临床意义
Expression and clinical significance of serum SIRT1 in patients with chronic heart failure

摘要(Abstract):

目的 探讨抗衰老酶1(SIRT1)与慢性心力衰竭(CHF)关系。方法 CHF患者107例(CHF组)和非心力衰竭患者59例(对照组)抽取早上空腹8 h的静脉血后,采用酶联免疫吸附法检测血清SIRT1水平,同时测定白细胞、中性粒细胞、血糖、肌钙蛋白T、肌酐、胆固醇、甘油三酯、丙氨酸氨基转移酶等血生化指标及N端脑钠肽前体(NT-pro BNP);采用IU33彩色多普勒超声诊断仪左室舒张末期内径(LVEDd)、左房前后径(LAD)、室间隔厚度(IVSD)、左室后壁厚(LVPW)及左心室射血分数(LVEF);用Spearman分析SIRT1与NT-pro BNP及心室结构指标的相关性。结果 CHF组肌钙蛋白T、肌酐升高,胆固醇及甘油三酯下降,差异有统计意义(P<0.05);CHF组患者血清SIRT1及NT-pro BNP指标均高于对照组患者,差异有统计意义(P<0.05);经Spearman相关分析显示,血清SIRT1与NT-pro BNP水平呈明显正相关,差异有统计意义(P<0.05);CHF组的LVEDd、LAD、LVPW及IVSD均较对照组增大,且与SIRT1水平呈正相关(P<0.05);CHF组LVEF较对照组明显降低,且与SIRT1水平呈负相关(P<0.05)。结论 CHF患者血清SIRT1表达水平明显升高,且与NT-pro BNP呈正相关,其对心脏可能有一定的保护作用。
Objective To investigate the relationship between serum sirtuin 1(SIRT1) and chronic heart failure(CHF). Methods One hundred and seven patients with CHF(CHF group) and 59 patients without heart failure(control group) were sampled for 8h morning fasting venous blood. Serum SIRT1 levels were detected by enzyme-linked immunosorbent assay. Blood biochemical indexes such as white blood cells, neutrophils, blood glucose, troponin T, creatinine, cholesterol, triglyceride, alanine aminotransferase, and N-terminal pro-brain natriuretic peptide(NT-Pro BNP) were tested; IU33 Color Doppler ultrasonography was used to measure the left ventricular end-diastolic diameter, left ventricular end-diastolic dimension(LVEDd), left atrial diameter(LAD), interventricular septal thickness(IVSD), left ventricular posterior wall thickness(LVPW), and left ventricular ejection fraction(LVEF); Spearman analysis was adopted to analyze the correlation between SIRT1 and NT-Pro BNP as well as the ventricular structural indexes. Results In CHF group, troponin T and creatinine were significantly increased, while cholesterol and triglyceride were significantly decreased, differences were statistically significant(P<0.05); levels of serum SIRT1 and NT-pro BNP indexes in CHF group were significantly higher than those of control group, differences were statistically significant(P<0.05). Spearman correlation analysis indicated that there was a significant positive correlation between serum SIRT1 and NT-pro BNP(P<0.05). LVEDd, LAD, IVSD, and LVPW in CHF group were higher than those of control group(P<0.05), which were positively correlated with the level of SIRT1. LVEF in CHF group was significantly lower than that of control group(P<0.05), but negatively correlated with the level of SIRT1. Conclusion The expression level of serum SIRT1 in CHF patients is significantly increased, which is positively correlated with NT-Pro BNP. It can be inferred that it owns certain protective effect on the heart.
