碘超足量地区碘营养状况对自身免疫性甲状腺疾病的影响
Influence of Iodine Nutrient Status on Autoimmune Thyroid Disease in Iodine Super Excessive Area

摘要(Abstract):

目的:探讨碘超足量地区居民碘营养状况对自身免疫性甲状腺疾病(AITD)的影响。方法:贵阳市1 492例(男性627例,女性865例)资料齐全的社区人群,采集静脉血分离血清检测促甲状腺激素(TSH)、甲状腺过氧化物酶抗体(TPOAb)、甲状腺球蛋白抗体(TGAb)、血清25羟维生素D水平[25(OH)D]、甲状旁腺激素(i PTH)、血钙及血磷水平,TPOAb和(或)TGAb阳性者定义为AITD;收集尿液检测尿碘水平,将人群分为碘缺乏组、碘正常组、碘超足量组及碘过量组,分析碘营养状况与AITD的关系。结果:总体人群尿碘水平为(238.95±162.03)μg/L,贵阳属于碘超足量地区;碘缺乏组具有较高的TSH水平,碘缺乏组AITD患病率较碘超足量或碘过量组高(P<0.05)。结论:贵阳地区的碘营养缺乏人群具有较高AITD患病率。
Objective: To investigate influence of iodine nutrient status on autoimmune thyroid disease of habitants in iodine super excessive area. Methods: A total of 1 492 local people(627 male,865 female) in Guiyang were performed collecting venous blood to separate serum for testing on TSH,TPO-Ab,i PTH,blood calcium and serum photsphate level; TPO-Ab and( or) TGAb positive were defined as AITD; collecting urine for urinary iodine level testing. Dividing participants into iodine deficiency group,iodine normal group,iodine super excessive group and iodine excessive group,analyzing interrelation between iodine nutrient status and AITD. Results: General urinary iodine level was(238. 95 ± 162. 03) μg /L,iodine deficiency group showed relatively higher TSH level,and AITD morbidity of this group was higher than super excessive group and excessive group( P < 0. 05). Conclusions: Guiyang is proven as iodine super excessive area,iodine deficiency population showed higher AITD morbidity.
