院外营养干预对胃癌术后患者生活质量的影响The Effect of Out-of-Hospital Nutritional Intervention on Quality of Life of Patients with Gastric Cancer after Operation

摘要(Abstract):
目的:探讨院外营养干预对胃癌术后患者生活质量的影响。方法:108例胃癌术后患者随机分为对照组(n=53)和试验组(n=55),分别给予常规出院指导和个性化营养指导,收集2组患者出院前(干预前)和出院后(干预后) 1个月的基本资料、体质量(BW)和体质量指数(BMI),并采用欧洲癌症研究与治疗组织生活质量核心30量表(EORTC QLQ-C30)对2组患者干预前和干预后1个月的生活质量(从总健康状况、功能、症状三个领域及6个单一条目)进行评价。结果:试验组患者干预后BW和BMI变化值均低于对照组(P <0.05); 2组患者干预后总健康状况、躯体功能、角色功能、认知功能、情绪功能、社会功能、恶心呕吐及食欲丧失均分均较干预前上升,而疲倦、疼痛、气促及便秘评分较干预前降低(P <0.05);试验组患者干预后总健康状况、躯体功能、角色功能、认知功能、情绪功能及社会功能评分均高于对照组,而疲倦、疼痛、恶心呕吐及气促评分评分均低于对照组(P <0.05)。结论:院外营养干预有助于改善胃癌术后患者的生活质量。
Objective: To investigate the effect of nutritional intervention on the life quality of patients with gastric cancer after gastrectomy. Methods: 108 subjects with gastric cancer after gastrectomy were randomly divided into the control group( n = 53) with normal nutritional guidance and the trial group( n = 55) with individualized nutritional intervention,and follow-up visits were conducted in both groups. The information of body weight( BW),body mass index( BMI) and life quality of the subjects was collected before discharge and one month after discharge respectively. The basic information of subjects was collected before discharge. Results: The changes of BW and BMI in the trial group after intervention were lower than those in the control group( P < 0. 05). The assessment scores of total health status,physical function,role function,cognitive function,emotional function,social function,nausea and vomiting and appetite totally increased after intervention,but the scores of exhaustion, pain, breathlessness and constipation descended in both groups, and the difference is statistically significant( P < 0. 05). The total health status,physical function,role function,cognitive function,emotional function,and social function scores in the trial group after intervention were higher than those in the control group,while fatigue,pain,nausea,and vomiting and breathlessness scores were lower than in the control group,and the difference was statistically significant( P < 0. 05). Conclusion: The nutritional interventions conducted out of hospital contributes to the improvement of life quality of gastric cancer patients with gastrectomy.
