心率变异性的时间稳定性研究

摘要(Abstract):

随机对 4 0例冠心病患者 (包括 2 0例心绞痛型患者 ,2 0例陈旧性心肌梗塞患者 )及 2 1例正常人 ,进行 2次Holter记录 ,分析心率变异性 (HRV )的时域及频域指标。结果表明正常人群HRV在较高水平上保持时间稳定性 ,冠心病人群在较低水平上保持时间稳定性 ,器质性心脏病的存在 ,并不增加HRV日 -日变异程度 ,这可大大提高HRV作为猝死预测指标的可靠性。冠心病心绞痛型及陈旧性心肌梗塞型病人 ,在服药一致的情况下 ,HRV进行性降低 ,可能是心脏事件发生的预兆。HRV降低 ,同时出现HRV的时间稳定性 (SOT)被打破 ,可能更强烈预示心脏事件发生的可能性 ;HRV未降低 ,HRV的SOT良好则预示不易发生心脏事件
Both time and frequency domain measures of heart rate variability (HRV) have been used to assess the tone of autonomic nerve in a variety of clinical conditions.HRV appears to be a strong predictor of sudden death.Few studies have been performed to determine the stability over time of heart rate variability in our country .20 patients with angina pectories of CHD,20 patients with old myocardial infarction of CHD,and 21 normal subjects were studied.For each subject two 24-hour dynamic electrocardiographic recordings were observed,time and frequency domain measures of HRV were analyzed.Stability over time of HRV is excellent in normal subjects and patients with CHD,it can improve it's value for predicting sudden death.In clinical unchanged conditions, the day-to-day variability of these measurements of HRV makes it possible to detect small changes due to progression of disease.A reduction in HRV and disturbing of HRV's stability over time may strongly predict sudden cardiac death.These results suggest that sequential measurement of HRV may be useful in predicting sudden death.More patients will be studied to confirm this finding in our hospital.
