新型生物可吸收聚合物弹簧圈栓塞颅内动脉瘤实验研究

摘要(Abstract):

目的:评价新型生物可吸收聚合物弹簧圈M atrix栓塞犬实验性动脉瘤的长期组织反应和安全性。方法:6条犬双侧颈总动脉建立12个实验性动脉瘤,一侧6个动脉瘤用M atrix栓塞,另一侧6个动脉瘤为对照。分析栓塞后14 d、1个月和8个月的造影和组织病理学资料。结果:对照侧动脉瘤一直开通,瘤内无血栓形成,瘤颈无新生内膜覆盖;M atrix栓塞侧动脉瘤尽管并非致密填塞,未观察到再通或破裂;栓塞后14 d瘤腔80%以上的区域形成机化血栓,瘤颈口被薄层新生内膜覆盖。栓塞后1个月瘤腔血栓完全机化,瘤颈新生内膜增厚;栓塞后8个月动脉瘤随瘢痕组织收缩而缩小。结论:M atrix能加速犬实验性动脉瘤纤维化和新生内膜形成而不引起载瘤动脉狭窄或血栓形成;应用M atrix栓塞颅内动脉瘤可能预防再通和改善长期解剖治愈结果,动脉瘤缩小可以减轻占位效应。
Objective: To evaluate the long-term tissue response to and safety of the new matrix coil system for embolization of intracranial aneurysms in dog.Methods:A new embolic agent,matrix coils,consists of thin platinum coils covered with a kind of bioabsorbable polymeric material(polyglycolic acid/lactide).Twelve experimental aneurysms were microsurgically created in 6 mongrel dogs(2 in each).In each animal,only one aneurysm was embolized with Matrix coils and the other one served as control.Comparative angiographic and histopathologic data were analyzed on the 14~(th) day(n=2),and in 1 month(n=2),and 8 months(n=2) respectively after embolization.Results: The control aneurysms were clear and no thrombosis or no neointima covering the neck was found until animals' sacrifice.While in the matrix embolized aneurysms,no recanalization or rupturing was observed despite no tight packing was there.On the 14~(th) day after embolization,the treated aneurysms exhibited organized thrombus in more than 80% areas,and a thin neointima covered the necks.In 1 month post-embolization,the thrombus was completely organized,and the neointima became thicker.In 8 months pos-tembolization,the retraction of scar tissue decreased the sizes of aneurysms.Conclusions: In experimental dog aneurysms, matrix can accelerate aneurysm fibrosis and neck neointima formation without causing parent artery stenosis or thrombosis.The use of matrix may prevent aneurysm recanalization,and improve the long-term anatomical healing results.The diminishing size of aneurysms may decrease mass effect.
