丙肝患者血清HCV RNA和ALT的相关性Correlation Study of Serum HCV,RNA and ALT in Patients with Hepatitis C

摘要(Abstract):
目的:探讨丙型肝炎病毒抗体(anti-HCV)阳性人群血清肝炎病毒核糖核酸(HCV RNA)和丙氨酸氨基转移酶(ALT)的关系。方法:取334例抗-HCV阳性患者血清,采用酶联免疫吸附试验(ELISA)法检测抗-HCV,荧光定量PCR法检测HCV RNA,全自动生化检测仪测定ALT水平,观察HCV RNA阳性组和HCV RNA阴性组患者血清ALT水平,同时对HCV RNA低载量组、HCV RNA中载量组及HCV RNA高载量组的ALT水平异常率进行Logistic回归分析。结果:在抗-HCV阳性血清样本(334例)中,HCV RNA载量阳性226例(67.7%),HCV RNA阴性108例(32.3%);HCV RNA阳性组患者ALT水平和异常率均高于HCV RNA阳性组患者(P<0.05);HCV RNA低载量组、HCV RNA中载量组及HCV RNA高载量组的ALT异常率分别为33.3%、69.5%、74.4%,ALT均值分别为100.7、103.0及98.6 U/L;Logistic回归分析结果显示,HCV RNA载量与ALT异常率呈正相关(r=0.874,P<0.05);HCV RNA载量与ALT水平变化无明显相关性(r=0.469,P>0.05)。结论:ALT异常率与HCV RNA载量呈正相关,而ALT水平与HCV RNA载量无相关性。
Objective: To investigate the relationship between serum hepatitis B virus ribonucleic acid( HCV RNA) and alanine aminotransferase( ALT) in( anti-HCV) positive population. Methods: The serum of 334 anti-HCV positive patients was extracted by by enzyme linked immunosorbent assay( ELISA) detecting anti-HCV,fluorogenic quantitative PCR detecting HCV RNA and automatic biochemical analyzer detecting the level of ALT to observe the level of serum ALT in the HCV RNA positive group and the HCV RNA negative group. Meanwhile,the abnormal rate of ALT level in HCV RNA low load group,HCV RNA load group and HCV RNA high load group was analyzed by Logistic regression. Results: 226 cases( 67. 7%) of HCV RNA load were positive; 108 cases of HCV RNA were negative( 32. 3%) in anti-HCV positive serum of 334 cases. The ALT level and abnormal rate of HCV RNA positive group were higher than those of HCV RNA positive group( P ﹤ 0. 05). The ALT abnormal rates in HCV RNA low load group,HCV RNA middle load group and HCV RNA high load group were respectively 33. 3%,69. 5% and 74. 4%. The ALT mean values were respectively 100. 7,103. 0 and 98. 6 U/L. The results of Logistic regression analysis showed the HCV RNA load was positively correlated with the abnormal rate of ALT( r = 0. 874,P ﹤ 0. 05) and HCV RNA load wasn't obviously correlated with the level of ALT( r = 0. 469,P ﹥ 0. 05). Conclusion: The abnormal rate of ALT was positively correlated with the HCV RNA load,while the level of ALT wasn't obviously correlated with HCV RNA load.
