奥沙西泮对3种乙醛脱氢酶2基因型酒精依赖综合征患者的治疗效果Efficacy of oxazepam in treating 3 types of aldehyde dehydrogenase-2 gene carriers in patients with alcohol dependence syndrome

摘要(Abstract):
目的 探讨奥沙西泮对3种乙醛脱氢酶2(ALDH2)基因型酒精依赖综合征(ADS)的治疗效果。方法 106例行奥沙西泮治疗的ADS患者根据ALDH2基因分型分为基因1型组((ALDH2×1/×1,n=47)、基因2型组(ALDH2×1/×2,n=35)及基因3型组(ALDH2×2/×2,n=24);比较3组患者入院和治疗第7天时的酒精依赖戒断综合征(AWS)评分、焦虑自评量表(SAS)评分、匹兹蛋睡眠质量指数(PSQI)评分及血清谷丙转氨酶(ALT)、谷草转氨酶(AST)、碱性磷酸酶(ALP)、谷酰转肽酶(GGT)水平,比较3组患者出院半年时的复饮率和再住院率。结果 3组患者治疗第7天时的AWS评分、SDS评分、SAS评分、PSQI评分及血清ALT、AST、ALP、GGT水平均较治疗前降低,差异有统计学意义(P<0.05);且在治疗前及治疗第7天时,3组患者AWS评分、SDS评分、SAS评分、PSQI评分及血清ALT、AST、ALP、GGT水平比较,基因3型组>基因2型组>基因1型组,差异有统计学意义(P<0.05);出院半年后,基因1型组和基因2型组ADS患者复饮率低于基因3型组,基因1型组再住院率ADS患者降低基因3型组,差异有统计学意义(P<0.05)。结论 奥沙西泮对3种ALDH2基因型ADS患者的治疗均有效,但治疗效果存在差异。
Objective To investigate the clinical efficacy of oxazepam in treating 3 types of aldehyde dehydrogenase-2(ALDH2) gene carriers in patients with alcohol dependence syndrome( ADS).M ethods One hundred and six patients with ADS were treated with oxazepam. According toALDH2 genotyping, all patients were divided into Geno type-1 group(ALDH2 × 1/× 1,n= 47), Geno type-2 group(ALDH2 × 1/× 2,n= 35), and Geno type-3 group(ALDH2 × 2/× 2,n= 24). The following indexes of three groups first admitted to hospital and 7 d after treatment were compared: AWS score,SAS score, and PSQI score, as well as levels of alanine aminotransferase( ALT), aspartate aminotransferase( AST), alkaline phosphatase( ALP), and r-glutamyle transpeptidase( GGT); in addition, re-drinking rate and rehospitalization rate of 3 groups were compared.Results The AWS score, SDS score, SAS score, and PSQI score, as well as serum indexes(ALT, AST, ALP, and GGT levels) of 3 groups 7 days after treatment were significantly lower than those before treatment,differences were statistically significant(P< 0. 05). Before and on 7 thday after treatment, comparison of AWS score, SDS score, SAS score, and PSQI score as well as serum indexes(ALT, AST, ALP,and GGT levels) showed Geno type-3 group > Geno type-2 group > Geno type-1 group, differences were statistically significant(P< 0. 05). After discharged from hospital for half a year, Geno type-1 and type-2 group showed lower re-drinking rate than that of Geno type-3 group, Geno type-1 group showed lower rehospitalization rate than that of type-3 group, differences were statistically significant(P< 0. 05).Conclusions Oxazepam is effective in the treatment of 3 types ofALDH2 gene carriers in patients with ADS, while efficacy shows differentiated results.
