重症肺炎患儿支气管肺泡灌洗液中IL-6、IL-10、IL-17及HMGB1水平
Study on Levels of IL-6,IL-10,IL-17 and HMGB1 in the Bronchoalbeolar Lavage Fluid of Children with Severe Pneumonia

摘要(Abstract):

目的:探讨重症肺炎患儿支气管肺泡灌洗液(BALF)中IL-6、IL-10、IL-17及高迁移率族蛋白B1(HMGB1)水平与病情严重程度及病程的关系。方法:选取住院治疗并行支气管肺泡灌洗术的重症肺炎患儿100例,将胸部CT提示受累肺叶≥2/3设定为A组、肺叶受累<2/3设定为B组,根据病程再将A、B两组分别分为A1组和B1组(急性期患儿,病程≤7 d)、A2组和B2组(恢复期患儿,病程>7 d);ELISA法检测BALF中IL-6、IL-10、IL-17及HMGB1水平,比较各组上述炎症因子水平。结果:A组IL-17水平高于B组,差异有统计学意义(P<0.05);、A1和B1组IL-6、IL-17水平高于A2和B2组,差异有统计学意义(P<0.05);A1组IL-6水平高于B1组,差异有统计学意义(P<0.05)。结论:BALF中IL-6、IL-17可作为反映急性期重症肺炎患儿病情严重程度的有用指标。
Objective: To explore the relationship between the level of IL-6,IL-10,IL-17 and HMGB1 in the bronchoalveolar lavage fluid( BALF) and severity and course of the disease. Methods:A total of 100 cases of severe pneumonia children treated with bronchoalveolar lavage were divided into group A( pulmonary lobe involvement no less than 2/3) and group B( pulmonary lobe involvement less than 2/3) according to chest CT. Then according to the course,group A and group B were further divided into two groups : group A1 and group B1( children with acute phase,course ≤7 d),group A2 and B2( children with recovery period,course > 7 d). The levels of IL-6,IL-10,IL-17 and HMGB1 in BALF were detected by Elisa,and the levels of the above mentioned inflammatory factors in each group were compared. Results: The ILl-17 level of group A was higher than that of group B,and the difference was statistically significant( P < 0. 05). The levels of IL-6 and IL-17 of group A1 and B1 were higher than those of group A2 and B2,and the differences were statistically significant( P <0. 05). The IL-6 level of group A1 was higher than that of group B1,and the difference was statistically significant( P < 0. 05). Conclusion: IL-6 and IL-17 in BALF can be used as useful indexes to reflect the severity of severe pneumonia in children with acute pneumonia.
