甲状腺激素水平与2型糖尿病预后的关系

摘要(Abstract):

目的:探讨2型糖尿病(T2DM)患者血清甲状腺激素水平与糖尿病病情的关系。方法:178例T2DM患者作为糖尿病组,同时期117例健康体检人群作为对照组,分别采集糖尿病组入院时及对照组体检时清晨空腹静脉血4 m L,采用化学发光法检测游离三碘甲状腺原氨酸(FT3)、游离甲状腺素(FT4)及促甲状腺激素(TSH)水平,采用免疫比浊法检测血清糖化血红蛋白(Hb A1c)水平;T2DM患者按Hb A1c水平和并发症再次分组,比较各组间FT3、FT4及TSH改变。结果:糖尿病组与正常对照组比较,FT3水平明显降低(P<0.01),FT4、TSH水平变化不明显(P>0.05);糖尿病患者随着Hb A1c水平增加,仅FT3呈下降趋势,组间比较差异有统计学意义(P<0.05);糖尿病并发症患者FT3水平显著低于无并发症患者(P<0.05或P<0.01)。结论:T2DM患者常伴有FT3水平降低,尤其对血糖控制不佳和有并发症的患者,FT3降低更明显。
Objective: To investigate the relationship between Type 2 diabetes mellitus with thyroid hormone( FT3、FT4、TSH) level change. Methods: A total of 178 patients with type 2 diabetes as diabetes group,117 healthy people as control group. Extracting venous blood of 2 groups,using chemiluinescence detection of serum thyroid hormone levels. Comparing two groups free three iodine thyroid glycine( FT3),free thyroxine( FT4) and thyroid stimulating hormone( TSH) changes; in patients with type 2 diabetes adopting immunoturbidimetric assay to detect Hb A1 c level. T2 DM patients will be regrouped according to Hb A1 c level and complications,in order to compare changes of FT3,FT4 and TSH. Results: Comparing Type 2 diabetes group with normal control group,FT3 level significantly decreased( P < 0. 01); no obvious difference was found between FT4 and TSH levels( P > 0. 05). With the increase of concentration of Hb A1 c,only FT3 level decreased( P < 0. 05 or P < 0. 01),FT3 level of diabetes complication group was obviously lower than no complication group( P < 0. 05 or P < 0. 01).Conclusion: Patients with type 2 diabetes often show decreased FT3 level,and patients with poor control of blood sugar and complications will lead obvious FT3 decrease.
