煅烧骨与羧甲基壳聚糖复合材料修复家兔下颌骨缺损的成骨效果

摘要(Abstract):

目的:探讨煅烧骨/羧甲基壳聚糖(CB/CS)复合材料修复骨缺损的成骨效果。方法:制备家兔下颌骨缺损动物模型,分别将煅烧骨、Bio-oss骨粉、CB/CS复合材料、Bio-oss/羧甲基壳聚糖复合材料植入家兔下颌骨缺损处,并设不植入任何材料的空白对照组,每组12只;分别于术后第2、4、8及12周时处死家兔(每次每组各3只),取出术区标本观察骨缺损区大体结构变化,比较Micro-CT检测的骨密度(BMD)、骨小梁厚度或孔壁厚度(Tb.Th)、骨小梁分离度或孔隙率(Tb.Sp)及骨小梁数量(Tb.N)值评价成骨效果。结果:肉眼可见各时间段CB/CS组家兔的骨缺损愈合程度高于其其他各组家兔、新生骨质多于其他各组家兔,Micro-CT检测结果显示,术后第2、4、8及12周时各组BMD、Tb.Th、Tb.Sp及Tb.N值比较,差异有统计学意义(P <0.05),CB/CMC组在各时间段BMD、Tb.Th值均显著高于煅烧骨组、Bio-oss骨组及空白组,术后第2周开始CB/CMC组Tb.Sp值显著低于空白组和其他几组,术后第4、8、12周时CB/CMC组Tb.N值显著高于其他各组(P <0.05)。结论:CB/CS复合材料在植入动物体内后的12周内,能很好地修复骨缺损。
Objective: To investigate osteogenic effect of calcined bone/carboxymethyl chitosan composite in repairing rabbit mandibular defect. Methods: The rabbit mandibular defect animal model was established. Calcined bone, Bio-oss bone meal, CB/CS composite material and Bio-oss/Carboxymethyl Chitosan composite material were implanted into the rabbit mandibular defect,while the blank control group was not implanted. At 2(nd),4(nd),4( th),8( th),8*th) and 12*th) and 12( th) weeks after surgery,3 rabbits/group were sacrificed. The general structural changes of the bone defect areas in specimens of the operation areas and evaluated the osteogenesis effect via comparing BMD measured by Micro-CT,trabecular bone thickness or hole wall thickness( Tb. Th),trabecular bone separation or porosity( Tb. Sp) and number of trabecular bones( Tb. N) were observed. Results: CB/CS group had the best healing degree of bone defects and the most new bone mass. Micro-CT analysis revealed that BMD,Tb. Th and Tb. N of the Calcined bone/Carboxymethyl Chitosan composite material were significantly different at2( th) weeks after surgery,3 rabbits/group were sacrificed. Sp values in CB/CMC group were the lowest since 2 ndweek after the surgery. In addition,Tb. N values of the CB/CMC group was the highest at4(th),8(th),8(th)and 12(th)and 12(th)weeks after surgery( P < 0. 05). Conclusion: CB/CS composite materials can repair bone defects well within 12 weeks after implantation in animals.
