玉屏风颗粒、氯雷他定联合孟鲁司特对支气管哮喘患儿气道重塑及IL-5、IFN-γ、CEC水平的影响Effect of Yupingfeng granules,loratadine combined with montelukast on airway remodeling and levels of IL-5,IFN-γ,and CEC in children with bronchial asthma

摘要(Abstract):
目的 探讨玉屏风颗粒、氯雷他定联合孟鲁司特对支气管哮喘患儿的治疗效果及气道重塑、白细胞介素-5(IL-5)、干扰素-γ(IFN-γ)、内皮细胞(CEC)水平的影响。方法 136例支气管哮喘患儿均分为对照组和观察组,对照组患儿采用氯雷他定联合孟鲁司特钠治疗,观察组患儿采用氯雷他定联合玉屏风颗粒治疗,比较2组患儿治疗4周时的治疗有效率,比较2组患儿治疗前和治疗4周时的气道平滑肌、气管内壁、气管管壁厚度,血清IL-5、IFN-γ及CEC水平,用药期间的不良反应发生率及随访1、3及6个月时的支气管哮喘复发率。结果观察组患儿治疗4周时的治疗有效率(97.06%)高于对照组(86.76%,P <0.05),观察组患儿治疗4周时的平滑肌、气管内壁、气管管壁厚度低于对照组,血清IL-5、CEC水平低于对照组,IFN-γ水平高于对照组,差异有统计学意义(P <0.05); 2组患儿用药期间不良反应发生率及随访1、3个月的复发率比较,差异无统计意义(P> 0.05);观察组随访6个月复发率低于对照组,差异有统计学意义(P <0.05)。结论 玉屏风颗粒、氯雷他定联合孟鲁司特用于支气管哮喘患儿,能改善气道重塑,降低IL-5、IFN-γ、CEC水平,降低6个月复发率。
Objective To expolre the clinical effect of Yupingfeng granules,loratadine combined with montelukast on airway remodeling and levels of interleukin-5 (IL-5),interferon-γ (IFN-γ),and endothelial cell (CEC) in children with bronchial asthma.Methods A total of 136 children with bronchial asthma were divided into control and observation groups.Control group was treated with loratadine combined with montelukast,and observation group was treated with Yupingfeng granules and loratadine combined with Yupingfeng granules.Treatment effectiveness was compared at 4 weeks o treatment between two groups.Airway remodeling levels of airway smooth muscle,tracheal inner wal and tracheal wall thickness,the levels of serum IL-5,IFN-γ,and CEC as well as the incidence o adverse reactions during medication at 4 weeks of treatment were compared between two groups.Recurrence rates of bronchial asthma at 1-,3-,and 6 month-follow-ups were compared between two groups.Results After 4 weeks of treatment,the effective rate of observation group was higher than that of control group (97.06%versus 86.76%,P<0.05).Airway remodeling levels of airway smooth muscle,tracheal inner wall and tracheal wall thickness were lower in observation group than those in control group.The levels of serum IL-5 and CEC were less in observation group than those control group,while IFN-γ level was higher than that in control group (P<0.05).There were no differences in the incidence rates of adverse reactions during medication and the recurrence rates at 1-and 3-month-follow-ups between two groups (P>0.05).The recurrence rates at 6-month follow-up was lower in observation group than that in control group (P<0.05).Conclusion Yupingfeng granules,loratadine combined with montelukast can improve airway remodeling,reduce the levels of IL-5,IFN-γ,and CEC in children with bronchial asthma and decrease the recurrence rate at 6 month follow-up.
