共情能力对医患双方医患关系评价的影响
The Effect of Empathy Ability on Doctor-patient Relationship

摘要(Abstract):

目的:了解医患双方医患关系评价及共情能力情况,分析共情能力水平与医患关系评价的关系。方法:采用抽样法抽取贵阳市3所医院不同性别、年龄、文化程度的98名医护人员和109名患者(或其家属),发放问卷进行调查,PDRQ-15量表用于患者(或其家属)对当前医患关系的评价、DDPRQ-10量表用于医务人员对当前医患关系的评价、人际反应指针量表(IRI-C)评价医患双方的共情能力;比较医患关系评价和共情能力在人口学特征上的差异,同时用Pearson相关性检验与线性回归分析二者之间的关系。结果:DPRQ-10总分为(3194±563)分,PDRQ-15总分为(5950±1154)分,医护人员共情能力IRI-C得分(4042±1259)分,患者(或其家属)得分(4750±994)分;医护人员在医患关系评价和共情能力的评价上存在年龄差异,而患者在共情能力的评价方面有性别和文化程度差异;医护人员共情能力的观点采择、共情性关心、个人痛苦维度以及总分均与医患关系评价各维度及总分呈正相关;患者角度的患者对医生的满意度与共情性关心、个人痛苦、IRI-C总分呈现正相关,患者对医疗症状的态度与观点采择、个人痛苦、IRI-C总分呈现正相关,PDRQ-15总分与IRI-C总分呈正相关。结论:医患双方的医患关系评价与共情能力呈正相关,共情能力能在一定程度上预测医患关系。
Objective: To investigate the basic situation of doctor-patient relationship and their empathy ability, and analyze the correlation between the level of empathy ability and the evaluation of doctor-patient relationship.Methods: 98 cases of medical staff and 109 patients(or their families)from three hospitals in Guiyang China were selected and tested through Patient-Doctor Relationship Questionnaire and Interpersonal Reactivity Index-C. The differences in the demographic characteristics of doctor-patient relationship and empathy ability were compared, and the relationship between them was examined by Pearson correlation test and regression analysis.Results: The total points of DDPRQ-10 was(31.94 ±5.63), PDRQ-15 was(59.50 ±11.54), the total points of empathy of medical staff(IRI-C) was(40.42 ±12.59) and that of patients was(47.50 ±9.94). There were age differences in the assessment of doctor-patient relationship and empathy ability among medical staff, while gender and education differences were found in the evaluation of empathy ability among patients. For medical staff, perspective taking(PT),empathic concern(EC), personal pain dimension(PPD) and total scores of IRI-C were positively correlated with each dimension and total score of DDPRQ-10. For patients and their families, patient satisfaction with doctors was positively correlated with EC, PPD and total score of IRI-C; Patients' attitude toward medical symptoms was positively correlated with PT, PPD and total score of IRI-C;PDRQ-15 total score was positively correlated with IRI-C total score.Conclusion: The sensibility of doctor-patient relationship is positively correlated with the empathy ability,which can predict doctor-patient relationship to a certain extent. The harmonious doctor-patient relationship can be constructed through the cultivation and promotion of empathy ability.
