脑出血患者急性期血清胃泌素水平与应激性溃疡的关系The Relationship between Changes of Serum Gastrin Content and Stress Ulcer in Patients with Cerebral Hemorrhage in Acute Stage

摘要(Abstract):
目的:探讨血清胃泌素水平与脑出血并发应激性溃疡之间的关系。方法:根据有无消化道出血、脑出血部位、脑出血量分为合并消化道出血组、单纯脑出血组,非丘脑出血组、丘脑出血组,基底节区小量脑出血组、基底节区中大量脑出血组,用放免法测定各组(脑出血患者发病后24 h内)血清胃泌素水平,并与正常人作对照。结果:脑出血患者发病后24 h血清胃泌素水平较正常对照组明显增高(P<0.01),合并应激性溃疡出血者升高更显著(P<0.01),出血发生在丘脑者比发生在其它部位者血清胃泌素水平高(P<0.05),同一部位出血量大的患者血清胃泌素水平高于出血量小的病例(P<0.05)。结论:血清胃泌素在脑出血并发应激性溃疡的发生中起重要作用,脑出血后血清胃泌素水平与出血量及出血部位有关。
Objective: To investigate the relationship between the changes of serum gastrin levels(SGL) and cerebral hemorrhage complicated with stress ulcer,and to study whether stress ulcer correlates with bleeding location and blood volume.Methods: Patients were divided into cerebral hemorrhage combined with gastrointestinal bleeding group,cerebral hemorrhage alone group,non-thalamic hemorrhage group,thalamic hemorrhage group,small volume of basal ganglia hemorrhage group,large volume of basal ganglia cerebral hemorrhage group according to whether combined with gastrointestinal bleeding,cerebral bleeding site,and blood volumes.SGL of patients with cerebral hemorrhage in 24 hours after onset were measured with radioimmunoassay and compared with those in control group.The data obtained were analyzed with t test.Results: During the first 24 h after cerebral hemorrhage,SGL in all the diseased groups were significantly higher than those in control group(P<0.01)and much higher in patients with complicated stress ulcer(P<0.01).SGL in patients with thalamic hemorrhage or large volume bleeding were higher than those in patients with cerebral hemorrhage in other sites or small volume bleeding(P<0.05).Conclusions: Gastrin might play an important role in the genesis of stress ulcer in patients with cerebral hemorrhage.Stress ulcer is prone to happen in patients with SGL increasing after cerebral hemorrhage,especially in patients with thalamic hemorrhage and large volume bleeding.
