三种不同镍钛器械联合热牙胶在后牙弯曲根管预备中的效果
Clinical Evaluation of Three Rotary Nickel-Titanium Instruments Combined with Hot Teeth Glue in Preparation of Posterior Teeth Curved Root Canal

摘要(Abstract):

目的:比较3种不同镍钛器械联合热牙胶在后牙弯曲根管预备中的治疗效果。方法:52例(52颗)急慢性牙髓炎或根尖周炎并进行根管治疗患者随机分为PT组(n=17,采用Pro Taper预备根管)、PTN组(n=17,采用Pro Taper Next预备根管)及S3组(n=18,采用S3系统预备根管),均采用连续波热牙胶完成根管充填;比较3组患者根管预备时间、根管填充效果、治疗过程及随访期间患者的材料宿主反应及不良反应;采用视觉模拟评分法(VAS)评价术后24、48及72 h患者的疼痛情况。结果:与PT组比较,PTN组和S3组根管预备时间明显缩短(P<0.01);与S3组比较,PTN组根管预备时间明显缩短(P<0.05);3组患者根管适充、欠充、超充及适充率比较,差异无统计学意义(P>0.05);3组患者术后24 h疼痛发生率比较,差异无明显统计学(P>0.05);术后48及72 h时PTN组患者疼痛发生率最低,其次为S3组(P<0.05或P<0.01);治疗过程中及随访期间,3组均未发生治疗不良事件及材料宿主反应事件。结论:镍钛器械中的Pro Taper Next预备根管在后牙弯曲根管预备中治疗效果优于Pro Taper及S3系统预备根管。
Objective: To investigate the effect of three rotary nickel-titanium instruments combined with hot teeth glue in the preparation of posterior teeth curved root canal. Methods: 52 cases( 52 teeth) of patients with acute or chronic pulpitis or periapical periodontitis treated with endodontics were selected and randomly divided into 3 groups. Group PT( n = 17) adopted Pro Taper for root canal preparation,group PTN( n = 17) adopted Pro Taper Next for root canal preparation,group S3( n =18) adopted S3 root canal preparation,and three groups were treated with continuous wave of hot tooth glue to complete root canal filling. The preparation time and non-preparation time of root canal,the effect of root canal filling,response and adverse reaction from patients during treatment and follow-up period were compared among the 3 groups. The pain of the patients was evaluated by VAS at 24 h,48 h and 72 h after operation. Results: Compared with group PT,the preparation time of root canal in groups PTN and S3 was significantly shortened( P < 0. 01). Compared with group S3,the preparation time of root canal in group PTN significantly shortened( P < 0. 05). There was no significant difference in root canal filling rate among the 3 groups( P > 0. 05). At 24 h after operation,the incidence rate of pain in 3 groups was showed no significant difference( P > 0. 05); 48 h and 72 h after operation,the incidence of pain in group PTN was the lowest( P < 0. 05 or P < 0. 01). During the treatment and follow-up period,no adverse events and material host reaction events occurred in the 3 groups. Conclusion: The three rotary nickel-titanium instruments have good curative effect in the preparation of curved root canals. However,the Pro Taper Next system has significantly shorter expansion time,lower incidence of pain and showed better safety.
