结肠慢传输型便秘患者血浆胃动素的监测Monitoring on Plasma Motilin Levels and Colonic Motility in Patients with Colonic Slow Transit Constipation

摘要(Abstract):
目的 :通过对结肠慢传输型便秘 (STC)患者血浆胃动素 (MTL)含量的分析 ,探讨血浆MTL水平与STC患者结肠动力变化的关系。方法 :采用放射免疫法测定 2 1例STC患者和 33例正常人空腹及餐后 1h的血浆MTL水平 ,分析两者的差异 ,并将患者血浆MTL水平与结肠运输试验的标记物排出率做相关分析。结果 :2 1例STC患者的空腹和餐后 1h的血浆MTL含量分别为 (2 2 7 92± 90 4 6 )ng/L ,(2 6 7 94± 12 6 6 8)ng/L ,低于正常组的(2 88± 12 7 83)ng/L ,(336 16± 116 0 6 )ng/L。两者比较 ,差异有显著性 (P <0 0 5 ) ;STC患者餐后 1h的MTL含量与结肠运输试验的标记物排出率呈线性正相关 (r =0 5 5 ,P <0 0 1)。结论 :MTL释放减少可能是STC发病机制中的一个重要环节。
Objective: To study the relationship between plasma motilin (MTL) levels and colon motility of patients with slow transit constipation (STC) through analyze the plasma MTL levels of the patients.Methods: Fasting and postprandial plasma MTL levels at 1 hour after a standardized meal in 33 healthy people and 21 STC patients were measured with radioimmunoassay; Plasma MTL levels of STC patients and ejection rates of the markers in colon transit test were analyzed for the linear-correlations.Results: The fasting and postprandial plasma MTL levels in 21 patients (227.92±90.46 and 267.94±126.68) were lower than those of healthy people (288±127.83, 336.16±116.06), with a significant difference between two groups at 1 hour after a meal (P<0.05). The plasma MTL level of STC patients was positively linear-correlated to the ejection rate of the marker in colon transit test (r=0.55, P<0.01).Conclusion: A reduced MTL release could be one important step to the pathogenic mechanism of STC.
