特发性肺纤维化患者病情程度与血清TGF-β1、STAT1水平的相关性

摘要(Abstract):

目的:分析特发性肺纤维化(IPF)患者病情程度与血清转化生长因子-β1(TGF-β1)及转录活化因子1(STAT1)的相关性。方法:120例IPF患者作为观察组,50例同期健康体检者作为对照组,观察组于入院时、对照组于体检当日检测血清TGF-β1、STAT1水平,次日检测动脉血氧分压(Pa O2)、动脉血二氧化碳分压(Pa CO2)、动脉血氧饱和度(Sa O2)水平及用力肺活量(FVC)、第一秒用力呼气流量(FEV1);采用Pearson法分析血清TGF-β1及STAT1水平与IPF患者病情严重程度的相关性。结果:与对照组比较,观察组患者血清TGF-β1水平显著升高,血清STAT1水平、Pa O2、Pa CO2、Sa O2、FVC及FEV1水平显著降低,差异有统计学意义(P <0. 05); Pearson相关分析结果显示,IPF患者血清TGF-β1水平与其Pa O2及Sa O2水平均呈负相关(r=-0. 396、P <0. 05,r=-0. 457、P <0. 001),血清STAT1水平与Pa CO2水平呈负相关(r=-0. 411,P <0. 001); IPF患者血清TGF-β1与其FVC水平呈正相关(r=0. 573,P <0. 001),与FEV1水平呈负相关(r=-0. 462,P <0. 001);血清STAT1与FVC呈负相关(r=-0. 364,P <0. 05)。结论:IPF患者血清TGF-β1及STAT1水平异常,且与患者血气指标及肺功能间均存在一定相关性。
Objective: To analyze the correlation between the degree of disease and serum transforming growth factor-β1( TGF-β1) and transcription activation factor 1( STAT1) in patients with idiopathic pulmonary fibrosis( IPF). Methods: 120 IPF patients were treated as observation group and 50 patients were treated as control group. The levels of serum TGF-β1 and STAT1 were measured in the observation group and the control group on the day of physical examination,and arterial blood oxygen partial pressure( Pa O2),arterial blood carbon dioxide partial pressure( PaCO2),arterial oxygen saturation( SaO2),forced vital capacity( FVC) and forced expiratory flow( FEV1) were tested in the first second. Pearson method was used to analyze the correlation between the levels of serum TGF-β1 and STAT1 and the severity of IPF patients ' condition. Results: Compared with the control group,the levelof serum TGF-β1 in the observation group increased significantly. The levels of serum STAT1,PaO2,PaCO2,SaO2,FVC and FEV1 decreased significantly,and the difference was statistically significant( P < 0. 05). Pearson correlation analysis showed that the serum TGF-β1 level of IPF patients was negatively correlated with their PaO2 and SaO2 levels( r =-0. 396,P < 0. 05; r =-0. 457,P < 0. 001). Serum STAT1 and PaCO2 levels were negatively correlated( r =-0. 411,P <0. 001). Serum TGF-β1 in IPF patients were positively correlated with their FVC levels( r = 0. 573,P < 0. 001) and negatively correlated with the level of FEVI( r =-0. 462,P < 0. 001). Serum STAT1 was negatively correlated with FVC( r =-0. 364,P < 0. 05). Conclusion: The levels of serum TGF-β1 and STAT1 of IPF patients are abnormal and correlated with blood gas index and lung function.
