骨髓增生异常综合征238例骨髓象与血象分析
MYELODYSPLASTIC SYNDROME: BLOOD PICTURES AND BONE MARROW FINDINGS IN 38 CASES

摘要(Abstract):

本文总结了38例MDS的血象与骨髓象特征:全血细胞减少与二系血细胞减少占83.4%,有16.6%表现为贫血,未见单独粒细胞减少或血小板减少者。73.6%的病例外周血见幼红细胞。各型MDS之骨髓有程度不一的三系增生异常,但以红系增生异常的情况较常见,RA与SRA两型常以此为主要表现。全组45%的病例红系尚有不同程度的巨幼样变。粒系以增生减低多见。半数病例见小巨核细胞,但小巨核在RA与SRA两型则不易见到。本组病例未见Ⅰ型与Ⅱ型原始细胞增多,而见早幼粒或幼单增多,此后转为急白,因而我们认为,除协作组规定的Ⅰ型与Ⅱ型原始细胞外,也应考虑早幼粒或幼单增多的意义。
Morphological changes in the peripheral blood (PB) and bone marrow (BM) are studied in 38 patients with myelodysplastic syndrome. Pancytoenia and cytopenia in 2 cell lines were seen in 84.3% of the patients, 16.6% had anemia only, however pure neutropenia or thrombocytopenia was not seen in this series. Erythroblast was demonstratrated in the peripheral blood smear in 73.6% of the cases Dysplastic changes of the BM were found in all 3 cell lineages, dyserythr- opoieses which was a dominated feature of the group RA and SRA in this series was more common than the others. In the BM megaloblastoid change was found in 45% of the patients. Hypocellularity of the granulo- eyric series in the BM was frequently seen. Micromegakaryocytes may be seen in one half of the patients except in group RA and SRA. The blast cells type I and II were not present in some patients of this series, but were replaced by increasing of promyelocytes or promonocytes and these patients developed into acute leukemia finally. According to the findings in our study, the significance of increasing of promyelocytes or promonocytes in the BM with MDS as a prognostic parameter should be emphasized.
