粒细胞集落刺激因子对冻融胚胎移植周期薄型子宫内膜的治疗效果

摘要(Abstract):

目的:研究粒细胞集落刺激因子(G-CSF)宫腔内灌注疗法对冻融胚胎移植(FET)时的薄型子宫内膜的疗效及妊娠结局的影响。方法:选取行FET治疗,且既往周期被诊断为薄型子宫内膜或本周期子宫内膜厚度≤7 mm的不孕症患者55例,25例为对照组,30例为观察组;两组患者均采用同样的激素替代方案治疗,观察组在激素替代治疗的基础上给予G-CSF宫腔内灌注,且观察组又根据子宫内膜厚度分为G-CSF灌注1次组、2次组和3次组;采用阴道B超于黄体酮转化日观察各组子宫内膜厚度,并比较妊娠结局。结果:与对照组比较,观察组周期取消率低,内膜更厚、胚胎种植率和临床妊娠率更高,差异具有统计学意义(P<0.05);与G-CSF 1次组比较,G-CSF 2次组患者的内膜厚度、胚胎种植率和临床妊娠率均明显提高(P<0.05)。结论:多次G-CSF宫腔灌注可改善激素替代周期冻胚移植的内膜厚度,提升胚胎种植率和临床妊娠率。
Objective: To investigate the effect of intrauterine injection of granulocyte colony-stimulating factor( G-CSF) on the improvement of thin endometrium and clinical outcomes during the hormone replacement cycle( HRT) of frozen-thawed embryo transfer( FET). Methods: Fifty-five patients with thin endometrium( ≤7 mm) undergoing FET were selected,and 25 case as control group,30 cases as observation group. All patients use the HRT for preparing the endometrium. Based on HRT,observation group was given G-CSF intrauterine injection,and according to endometrial thickness,patients in observation group were divided into injecting once,twice and third time groups. The endometrial thickness of each group was observed by transvaginal B ultrasound on the progesterone conversion day and pregnancy results were compared. Results: Compared with the control group,the observation group had lower cycle ablation rate,thicker endometrium,higher implantation rate and clinical pregnancy rate( P < 0. 05). 2 times intrauterine perfusion can significantly improve the endometrium thickness,implantation rate and clinical pregnancy rates compared with 1 time( P < 0. 05). Conclusions: Multiple intrauterine infusion with G-CSF can improve the endometrium thickness during HRT of FET and improve embryo implantation rate and clinical pregnancy rate.
