缩短DNT对急性脑梗死溶栓患者神经功能的影响
Effect of shortening DNT on reducing nerve function injury in thrombolytic patients with acute cerebral infarction

摘要(Abstract):

目的 分析急性脑梗死患者溶栓治疗过程中,缩短患者到院就诊至静脉溶栓时间(DNT)对患者神经功能的影响。方法 将86例急性脑梗死患者根据DNT分为研究组(DNT≤60 min,n=43)和对照组(DNT>60 min,n=43),对比两组患者的美国国立卫生研究院卒中量表(NIHSS评分、外周静脉血中神经元烯醇化酶(NSE)、中枢神经特异蛋白(S100β)、内皮素-1(ET-1)、血栓烷B2(TXB2)以及一氧化氮(NO),同时检测两组患者的氧化应激指标超氧化物歧化酶(SOD)、丙二醛(MDA),观察治疗结局以及不良反应。结果 治疗后,与对照组比较,研究组患者的NIHSS评分、NSE以及S100β降低(P<0.05),ET-1与TXB2较低,NO水平较高(P<0.05);治疗后,研究组的SOD高于对照组,MDA低于对照组(P<0.05);研究组患者的死亡率、致死性脑出血率以及不良反应均低于对照组(P<0.05)。结论 在急性脑梗死患者进行溶栓治疗时,缩短DNT能够减轻患者神经功能的损伤程度,改善血管内皮功能与氧化应激反应,且不良反应少。
Objective To investigate the effect of shortening the time from hospital arrival to intravenous thrombolysis(door-to-needle time, DNT) on the nerve of patients with acute cerebral infarction during thrombolytic therapy. Methods A total of 86 patients with acute cerebral infarction were divided into study group(DNT≤60 min, n=43) and control group(DNT>60 min, n=43) according to DNT. National Institutes of Health Stroke Scale(NIHSS), neuron enolase(NSE) in peripheral venous blood, central nerve-specific protein S100β, endothelin-1(ET-1), thromboxane B2(TXB2), and nitric oxide(NO) were compared between two groups. At the same time, the indexes of oxidative stress such as superoxide dismutase(SOD) and malondialdehyde(MDA) in two groups were measured. Treatment outcome and adverse reactions were observed. Results After treatment, NIHSS score, NSE, and S100β in study group were decreased relative to control group(P<0.05). The levels of ET-1 and TXB2 were less in study group than those in control group, while NO level was higher in study group than that in control group after treatment(P<0.05). After treatment, SOD in study group was higher than that in control group, while MDA was lower than that in control group(P<0.05). Mortality rate, fatal cerebral hemorrhage rate, and adverse reactions of patients in study group were lower than those in control group(P<0.05). Conclusion In thrombolytic therapy for patients with acute cerebral infarction, shortening DNT can reduce the degree of neurological damage, improve vascular endothelial function and oxidative stress reaction with fewer adverse reactions.
