常规超声、超声造影、钼靶X线、磁共振成像及联合检测对早期乳腺癌患者的诊断价值Diagnostic values of conventional ultrasound, contrast-enhanced ultrasound, mammography, magnetic resonance imaging and their combinations in diagnosing patients with early breast cancer

摘要(Abstract):
目的 探讨常规超声、超声造影(CEUS)、钼靶X线及磁共振成像(MRI)联合检测对早期乳腺癌患者的诊断价值。方法 收集均行常规超声、CEUS、钼靶X线、MRI检查的21例乳腺癌和20例乳腺良性病变患者的资料,以病理组织学为金标准,比较4种检查方法单独及联合应用对乳腺癌诊断的敏感度、特异度、阳性预测值、阴性预测值及准确性,制作受试者工作特征曲线(ROC)、计算曲线下面积(AUC)评估4种检查方式单独或联合应用对诊断早期乳腺癌的能力。结果 CEUS检查单独诊断早期乳腺癌的特异度、阳性预测值、阴性预测值及准确性最好,MRI检查的敏感度最好;4种影像学技术联合应用时对乳腺癌诊断的敏感度、特异度、阳性预测值、阴性预测值及准确性均高于两两组合、或3种影像学技术的组合方式,诊断效能最好。结论 常规超声、CEUS、钼靶X线及MRI 4种影像学技术联合应用对早期乳腺癌具有较高的诊断效能。
Objective To investigate the diagnostic values of conventional ultrasound, contrast-enhanced ultrasound(CEUS), mammography, magnetic resonance imaging(MRI) and their combinations in diagnosing patients with early breast cancer. Methods Data of 21 patients with breast cancer and 20 patients with benign breast lesions who underwent conventional ultrasound, CEUS, mammography, and MRI were collected. Their histopathology was used as the gold standard for diagnosis. The sensitivity, specificity, positive predictive value, negative predictive value, and accuracy of individual imaging technique alone and their combinations in the diagnosing breast cancer were compared. Receiver operating characteristic curve(ROC) was made to calculate the area under the curve(AUC) for evaluating the ability of the four imaging techniques alone and their combinations in diagnosing early breast cancer. Results The specificity, positive predictive value, negative predictive value and accuracy of CEUS alone were the best in diagnosing early breast cancer. The sensitivity of MRI was the highest. The sensitivity, specificity, positive predictive value, negative predictive value, and accuracy of 4 imaging technique combination in diagnosing breast cancer were higher than those of the combinations of two or three imaging techniques, showing the best diagnostic efficacy. Conclusion The combination of conventional ultrasound, CEUS, mammography and MRI has high diagnostic efficacy for early breast cancer.
