自我强化管理教育干预对PICC带管肿瘤患者心理健康及自理能力的影响
Effects of self-reinforcing management education intervention on mental health and self-care ability of tumor patients with PICC

摘要(Abstract):

目的 探讨自我强化管理教育干预模式对经外周静脉置入中心静脉导管(PICC)带管肿瘤患者心理健康及自理能力的影响。方法 150例PICC带管肿瘤患者随机均分为观察组和对照组,对照组予以常规健康教育,观察组在常规健康教育基础上予以自我强化管理教育干预;分别在干预前后通过焦虑自评表(SAS)和抑郁自评表(SDS)观察两组患者心理健康水平,通过自理能力测量量表(ESCA)评估自理能力,综合评定量表(GQOL-74)评估生活质量;统计干预后所有患者导管留置时间及一次性穿刺成功率,采用纽卡斯尔护理服务满意度量表(NSNS)评定干预后满意度。结果 与对照组比较,观察组心理健康水平各维度评分降低,自理能力各维度评分升高,生活质量各项评分升高,导管留置时间明显延长,差异有统计学意义(P<0.05);观察组一次性穿刺成功率和干预满意度高于对照组,差异有统计学意义(P<0.05);多因素logistic回归分析结果表明,体位及穿刺部位是PICC带管肿瘤患者一次性穿刺成功率的独立风险因素(P<0.05)。结论 自我强化管理教育干预有利于改善PICC带管肿瘤患者心理健康,提高其自理能力、生活质量、干预满意度,延长导管留置时间并提高一次性穿刺成功率。
Objective To investigate the effect of self-reinforcing management education intervention on mental health and self-care ability of tumor patients with peripherally inserted central catheter(PICC).Methods A total of 150 tumor patients with PICC was randomly divided into observation group and control group. The control group was given routine health education, and the observation group was given self-reinforcing management education intervention plus routine health education.Before and after intervention, the mental health level of both groups was observed by Self-rating Anxiety Scale( SAS) and Self-rating Depression Scale( SDS), self-care ability was evaluated by ESCA, and quality of life was evaluated by generic quality of life inventory-74( GQOL-74). The catheter keeping time and one-time puncture success rate of all patients after intervention were counted,and the post-intervention satisfaction was assessed by Newcastle Nursing Service Satisfaction Scale(NSNS).Results Compared with the control group, scores of mental health level of observation group obviously decreased, scores for self-care ability and quality of life for observation group obviously increased, and catheter keeping time in the observation group was significantly prolonged, differences were statistically significant(P< 0. 05). The one-time puncture success rate and intervention satisfaction of observation group were higher than those in the control group, difference was statistically significant(P< 0. 05). Multivariate logistic regression analysis showed that body position and puncture site were independent risk factors for one-time puncture success rate in tumor patients with PICC, difference was statistically significant(P< 0. 05).Conclusion Self-reinforcing management education intervention is beneficial to improve the mental health of tumor patients with PICC,effectively improves their self-care ability, quality of life, intervention satisfaction, prolongs the catheter keeping time, and improves the one-time puncture success rate.
