循环肿瘤细胞联合癌胚抗原检测在非小细胞肺癌中的临床价值

摘要(Abstract):

目的:探讨循环肿瘤细胞(CTC)联合癌胚抗原(CEA)检测在非小细胞肺癌(NSCLC)中的临床价值。方法:选取病理确诊NSCLC患者54例为研究组(其中Ⅰ~Ⅱ期19例、Ⅲ期14例及Ⅳ期21例),肺良性疾病患者21例为对照组,抽取2组患者清晨空腹静脉血5 m L,采用免疫磁珠法检测患者的血清CTC水平;抽取2组患者清晨空腹静脉血3 m L,采用酶联免疫吸附法检测患者的血清CEA水平; CTC与CEA联合检测只要有1项检测指标阳性则为联合检测阳性。结果:研究组患者外周血CTC和CEA的阳性率高于对照组(P <0. 05);研究组患者ROC曲线结果显示,CTC、CEA及二者联合检测的ROC曲线下面积分别为0. 828 (95%CI为0. 723~0. 905)、0. 738(95%CI为0. 622~0. 831)及0. 737(95%CI为0. 622~0. 832);研究组患者外周血CTC联合CEA检测的阳性率高于对照组(P <0. 05),研究组Ⅲ~Ⅳ期患者阳性率则高于研究组Ⅰ~Ⅱ期(P <0. 05);研究组Ⅰ~Ⅱ期患者外周血CTC联合CEA检测的阳性率高于对照组(P <0. 05)。结论:CTC联合CEA检测较单一检测更有助于辅助诊断早期(Ⅰ~Ⅱ期) NSCLC。
Objective: To discuss the clinic value of the detection of circulating tumor cells( CTC combined with carcinoembryonic antigen( CEA) in the treatment of non-small cell lung cancer( NSCLC). Methods: Fifty-four patients with NSCLC were selected as the research group( stage Ⅰ ~Ⅱwith 19 cases,stage Ⅲ with 14 cases and stage Ⅳ with 21 cases) and 21 patients with benign lung diseases as the control group. 5 m L of fasting venous blood was collected from two groups in the morning and serum CTC level was detected by immunomagnetic bead method. 3 m L fasting venous blood was extracted from 2 groups in the morning,and serum CEA level was detected by enzymelinked immunosorbent assay. The combined test of CTC and CEA adopted parallel test. As long as an indicator in the combined test was found as positive,it was regarded to be positive. Results: The positive rate of CTC and CEA in peripheral blood of the research group was higher than that of the control group( P < 0. 05). The ROC curve results of the research group showed that the areas under the curves of CTC,CEA and CTC combined detection were 0. 828( 95% CI: 0. 723 ~ 0. 905) and 0. 738( 95% CI: 0. 622 ~ 0. 831) and 0. 737( 95% CI: 0. 622 ~ 0. 832),respectively. The positive rate of CTC + CEA in peripheral blood of patients with stage Ⅲ-Ⅳ was higher than that of patients with stageⅠ ~ Ⅱ in the research group( P < 0. 05). The positive rate of peripheral blood CTC + CEA in stageⅠ ~ Ⅱ Patients in the research group was higher than that in the control group( P < 0. 05).Conclusion: CTC combined with CEA is more helpful for the diagnosis of early stage( stage Ⅰ ~ Ⅱ)NSCLC than single test.
