摘要(Abstract)：

目的 :观察发育性髋关节脱位病例在闭合复位治疗过程中股骨颈干角 (CA)和前倾角 (FNA)发育的变化规律。方法 :33例 (4 1髋 )在 3年闭合复位治疗过程中分 6个时间段采集系列X线片 ,测量、观察CA和FNA的动态变化 ,收集同龄同等数量正常髋部资料做对照。结果 :脱位髋关节经闭合复位CA和FNA的改变为 :小龄组CA 15 3 5 0°→ 131 88° ,减少 2 1 6 2°;FNA 4 8 91°→ 33 70° ,减少 15 2 1° ;平均每年各减少 7 2 0° ,5 0 7° ;大龄组CA15 8 6 7°→ 14 9 32°,减少 9 35° ;FNA 5 2 6 7°→ 4 2 18° ,减少 10 4 9° ;平均每年各减少 3 12°、3 5 0°。复位后CA和FNA的恢复快于对照组 ,且在恢复过程中 ,大龄组和小龄组间存在明显的差异。结论 :股骨头复位是刺激CA和FNA下降的主要因素 ,年龄是影响其下降的另一个重要因素。
Objective: To study the serial changes of cervicofemoral angle (CA) and femoral neck anteversion (FNA) during the close reduction treatment of developmental dislocation of hips (DDH). Methods: A series of X-ray films of 33 patients (41 affected hips) were taken during the 3 and more years of treatment which was divided into 6 intervals. The films were reviewed and the CA and FNA were measured and analyzed. Results: In young group, the CA decreased from 153.50° to 131.88°, with a reduction of 21.62° and the average reduction was about 7.2°per year; the FNA decreased from 48.91° to 33.70°, with a reduction of 15.21° and an average reduction of 5.07°per year. In elder group, CA decreased from 158.67° to 149.32°, with a reduction of 9.35° and an average reduction of 3.12° per year; the FNA decreased from 52.67° to 42.18°, with a reduction of 10.49° and an average reduction of 3.50°per year. All the changes occurred with a faster rate than those in control group. The reduction of the angles in young group was different from those in the elder group. Conclusions: The reduction of femoral head is the main stimulating factor in the changes of CA and FNA, and the age of the patient is one of the important factors.
