认知-行为干预对颞下颌关节紊乱患者的干预效果

摘要(Abstract):

目的:探讨认知-行为干预对颞下颌关节紊乱(TMD)患者延续性护理的干预效果。方法:选取TMD患者108例,随机均分为实验组和对照组,对照组患者采取基础治疗及常规护理,实验组在对照组的基础上通过微信互动平台、护士门诊延续性护理、电话随访及健康讲座等多种方式进行认知-行为干预;于基础治疗结束时及干预后2、4、6个月采用一般资料问卷、简易应对方式问卷(SCSQ)、焦虑自评量表(SAS)及成人心理压力量表(APSS)对2组患者进行心理评估,观察上述时点2组患者的积极应对、焦虑、心理压力、治疗效果及满意度。结果:基础治疗结束时2组患者SCSQ、SAS、APSS量表评分比较差异无统计学意义(P>0.05);干预后2、4、6个月时2组患者的SCSQ、SAS、APSS量表评分比较,差异均有统计学意义(P <0.05); 2组患者SCSQ得分随4个时间点的变化趋势不同,实验组呈持续上升趋势,对照组呈先下降后上升趋势,实验组高于对照组; SAS、APSS得分随4个时间点的变化均呈下降趋势,实验组低于对照组;治疗效果、满意度随4个时间点的变化均呈上升趋势,实验组高于对照组;干预后2个月与基础治疗结束时SAS得分比较,差异有统计学意义(P <0.05);干预后4个月与基础治疗结束时的SAS、APSS得分比较,差异有统计学意义(P <0.05);干预后6个月与基础治疗结束时比较,SCSQ、SAS及APSS得分比较,差异有统计学意义(P <0.05);基础治疗结束时2组患者治疗效果和满意度比较,差异无统计学意义(P>0.05),干预后2、4、6个月两组患者治疗效果和满意度与基础治疗结束时比较,差异有统计学意义(P <0.05)。结论:运用认知-行为干预对TMD患者进行延续性护理可有效提升患者心理状况、治疗效果及满意度。
Objective: To investigate the effect of cognitive-behavioral intervention on the continuous nursing of patients with temporomandibular joint disorder( TMD). Methods: 108 patients with TMD were randomly divided into experimental group and control group. In addition to the basic treatment and routine nursing of control group,the experimental group received continuous nursing of cognitive behavioral intervention for TMD patients through Wechat interactive platform,continuous nursing,follow-up phone calls and health lectures. At the end of basic treatment and 2,4,and 6 months after the intervention,general information questionnaire,simple coping style questionnaire( SCSQ),Selfrating Anxiety Scale( SAS) and Adult Psychology Stress Scale( APSS) were used to assess patients' psychological status. The positive coping, anxiety, psychological pressure, treatment effect and satisfaction of the two groups were observed at previously mentioned time. Results: There were no significant differences in SCSQ,SAS and APSS scores between the two groups at the end of basic treatment( P > 0. 05); after intervention,the differences of SCSQ,SAS and APSS scores in both groups were statistically significantly( P < 0. 05). The changes of SCSQ scores in both groups were different in 4 recorded time: the experimental group showed a continuous rising trend,the control group showed a downward then rising trend,and the value of experimental group was higher than that of control group; the SAS and APSS scores showed a downward trend with the changes of four recorded time,and the value of experimental group was lower than that of control group; the treatment effect and satisfaction degree showed an upward trend with the changes of 4 recorded time,and the value of experimental group was higher than that of control group. There were statistical significance in SAS scores differences between 2 months after intervention and the end of basic treatment( P < 0. 05);there were statistical significance in SAS and APSS scores differences between 4 months after intervention and the end of basic treatment( P < 0. 05); there were statistical significance in SCSQ,SAS and APSS scores differences between 6 months after intervention and the end of basic treatment( P <0. 05); at the end of the basic treatment,there was no statistical significance in the treatment effect and satisfaction difference between the two groups( P > 0. 05). After 2,4,and 6 months intervention,the treatment effect and satisfaction of the two groups were significantly different from that of end of the basic treatment( P < 0. 05). Conclusion: The cognitive-behavioral intervention for continuous nursing of TMD patients can effectively improve their psychological status,treatment effect and satisfaction.
