急性脑血管病患者心率变异性频域分析指数的改变
Changes in Heart Rate Variability and its Clinical Significance in Patients with Acute Cerebral Vascuar Disease

摘要(Abstract):

目的 :探讨急性脑血管病 (AcuteCerebralVascularDisease,ACVD)患者心率变异性 (HeartRateVari ability ,HRV)频域指标所反映的植物神经功能变化及其临床意义。 方法 :对 6 0例ACVD患者 (包括 30例脑梗塞 ,30例脑出血 ,其中 10例脑出血破入脑室 )及 30例正常人 ,进行 2 4h动态心电图检查 ,分析HRV的频域指标所反映的植物神经改变。结果 :ACVD患者的HRV频域指标明显低于对照组 (P <0 0 1) ;急性脑出血组的HRV频域指标显著低于脑梗塞组 (P <0 0 1) ;破入脑室的脑出血组较未破入脑室组的HRV频域指标显著降低 (P <0 0 1)。结论 :HRV频域分析可作为一项定时评价ACVD后交感和迷走神经受损程度的指标 ,并对ACVD预后判断有一定意义
Objective:To explore the changes in vegetative nerve function by heart rate variability (HRV)and its clinical significance in patients with acute Cerebral Vascular Disease(ACVD). Methods:24 hour ECG was recorded in 60 patients with ACVD including 30 cases with brain infarction and 30 cases wiht brain hemorrhage (in 10 of them,the hemorrhage was broken into the brain ventricles) and 30 normal subjects as control. The changes of vegetative nerve function reflected by the frequency domian of HRV were analyzed.Results:HRV in ACVD was significantly lower than that in controls,HRV in patients with acute cerebral hemorrhage was significantly lower than that in patients with brain infarction.HRV in patients with cerebral hemorrhage broken into brain ventricles waw singificantly lower than that in those without hemorrhage in the ventricles.Conclusion:HRV might be an index in evaluating the degree of the vegetative nerve injury after the onset AVCD and might be of significance in judging the prognosis of ACVD.
