传染性单核细胞增多症儿童外周血中TLR9 mRNA及γδ-TCR型DNT细胞的表达
Expression of TLR9 mRNA in Peripheral Blood Serum and DNT Cell of γδ-TCR in Child Patients with Infectious Mononucleosis

摘要(Abstract):

目的:探讨传染性单核细胞增多症(IM)儿童中外周血中Toll受体9(TLR9)、γδ-TCR型双阴性T细胞(DNT)的表达。方法:90例IM患儿作为研究对象,其中急性期50例作为急性期组、恢复期40例作为稳定期组,30例同期健康体检儿童作为对照组; IM患儿于入院时、对照组儿童于体检当日抽取外周血,采用流式细胞检测淋巴细胞表型[CD3+、CD3+CD8+、CD3+CD4+、CD4/CD8、CD3-CD(16+56)+、CD3-CD19+]及γδ-TCR型DNT细胞表达,采用实时荧光定量PCR检测TLR9 mRNA表达水平。结果:外周血CD3+、CD3+CD8+及TLR9mRNA水平比较,急性期组间>稳定期组>对照组,差异有统计学意义(P <0. 05),外周血CD3+CD4+、CD4/CD8、CD3-CD(16+56)+及CD3-CD19+水平比较,急性期组<稳定期组<对照组,差异有统计学意义(P <0. 05);对照组、稳定期组及急性期组儿童的外周血中均有DNT细胞表达,为(γ+δ)+TCR型,且急性期组>稳定期组>对照组,差异有统计学意义(P <0. 05)。结论:IM患儿机体免疫机制失衡,这种免疫失衡在IM急性期更为显著,主要表现为TLR9表达上调及特异性(γ+δ)+TCR型DNT细胞表达升高。
Objective: To investigate the expression of serum Toll-like receptor 9( TLR9) and γδ-TCR-double negative T cells( DNT) in children with infectious mononucleosis( IM). Methods: 90 children with IM were studied,of whom 50 were in acute phase,40 cases were in convalescent stage and 30 cases of healthy children were in control group. The peripheral blood of IM children was collected on the day of physical examination and the expression of lymphocyte phenotype[CD3~+,CD3~+CD8+,CD3~+CD4~+,CD4/CD8,CD3~-CD( 16 + 56)~+,and CD3~-CD19~+]and DNT cells of γδ-TCR type was detected by flow cytometry. The expression levels of TLR9 mRNA were detected by real-time fluorescence quantitative PCR. Results: Comparison of CD3~+,CD3~+CD8~+,and TLR9 mRNA levels in peripheral blood: acute period > stable group > control group,the difference was statistically significant( P < 0. 05). Comparison of CD3~+CD4~+,CD4/CD8,CD3~-CD( 16 + 56)~+,and CD3~-CD19~+levels in peripheral blood: acute group < stable group < control group,the difference was statistically significant( P < 0. 05). The expression of DNT cells in the peripheral blood of children in control group,stable group and acute group was( γ + δ) + TCR,acute group > stable group > control group,and the difference was statistically significant( P < 0. 05). Conclusion: The immune mechanism of IM children is out of balance,which is more significant in the acute stage of IM. The expression of TLR9 is up-regulated and the DNT level of specific( γ + δ) + TCR type is increased.
