外周血NLRP3 mRNA、NF-κB、PCT水平与COPD患者肺功能及肺动脉高压的关系
Relationship between NLRP3 mRNA, NF-κB, PCT levels in peripheral blood and pulmonary function and hypertension of patients with COPD

摘要(Abstract):

目的 探讨NOD样受体热蛋白结构域相关蛋白3信使RNA(NLRP3 mRNA)、核因子激活的B细胞的κ-轻链增强(NF-κB)、降钙素原(PCT)与慢性阻塞性肺疾病(COPD)患者肺功能及肺动脉高压的关系。方法 选取COPD稳定期患者47例、COPD急性加重期(AECOPD)未合并肺动脉高压患者47例及AECOPD合并肺动脉高压患者47例,以同期体检健康人群为对照组(n=47),抽取4组受检者空腹静脉血,采用酶联免疫吸附法检测血液NF-κB和PCT表达,采用聚合酶链式反应(PCR)法检测外周血淋巴细胞NLRP3 mRNA的表达;采用肺功能仪检测对照组、COPD稳定期组及AECOPD未合并肺动脉高压组受检者的肺功能指标[第一秒用力呼气容积(FEV_(1 ))、用力肺活量(FVC)、FEV1/FVC、FEV_1占预计值的百分比(FEV_1%pred)];采用超声心动仪测量AECOPD未合并肺动脉高压组和AECOPD合并肺动脉高压组受检者的肺动脉高压、右心房径及右心室径。结果 受检者外周血PCT、NLRP3 mRNA、NF-κB水平表现为AECOPD未合并肺动脉高压组>COPD稳定期组>对照组(P<0.05);AECOPD未合并肺动脉高压组患者血清NLRP3 mRNA、PCT、NF-κB表达水平较AECOPD合并肺动脉高压组减少(P<0.05);受检者PCT、NLRP3 mRNA、NF-κB与肺功能FEV_1%pred、FEV_1/FVC指标呈负相关(P<0.05),与肺动脉高压、右心房内径、右心室内径3个指标均呈正相关(P<0.05)。结论 外周血NLRP3 mRNA、NF-κB、PCT水平可作为COPD疾病的监测指标。
Objective To investigate the relationship between NOD-like receptor thermoprotein domain-related protein 3 messenger RNA(NLRP3 mRNA), nuclear factor-activated κ-light chain enhancement(NF-κB), procalcitonin(PCT), and pulmonary function and hypertension in patients with chronic obstructive pulmonary disease(COPD). Methods Patients with stable COPD, acute exacerbation of COPD(AECOPD) without pulmonary hypertension and AECOPD with pulmonary hypertension were selected as stable COPD group(n=47), AECOPD without pulmonary hypertension group(n=47), AECOPD with pulmonary hypertension group(n=47), and the healthy people in the same period as the control group(n=47). Fasting venous blood was collected from the four groups, and the expressions of serum NF-κB and PCT were detected by enzyme-linked immunosorbent assay. The expression of NLRP3 mRNA in peripheral blood lymphocytes was detected by polymerase chain reaction(PCR). The pulmonary function indexes(forced expiratory volume in one second(FEV_1), forced vital capacity(FVC), FEV_1/FVC, and the percentage of FEV_1 to predicted value(FEV_1%PRED) in the control group, stable COPD group and AECOPD without pulmonary hypertension group were detected by pulmonary function instrument. Pulmonary hypertension, right atrial diameter, and right ventricular diameter were measured by echocardiography in AECOPD group without pulmonary hypertension and AECOPD group with pulmonary hypertension. Results The levels of PCT, NLRP3 mRNA, and NF-κB in AECOPD patients without pulmonary hypertension group were higher than those in stable COPD group and the control group(P<0.05). The expression levels of NLRP3 mRNA, PCT and NF-κB in the AECOPD without pulmonary hypertension group were significantly lower than those in the AECOPD with pulmonary hypertension group(P<0.05). PCT, NLRP3 mRNA, and NF-κB were negatively correlated with FEV_1%PRED and FEV_1/FVC(P<0.05), and positively correlated with pulmonary hypertension, right atrial diameter, and right ventricular diameter(P<0.05). Conclusion NLRP3 mRNA, NF-κB, and PCT can be used as monitoring indexes of COPD.
