不同胎龄新生儿骨代谢血清标志物及IGF-I检测The Test and Clinic Significance of Bone Metabolism Biochemical Markers and IGF-I in Newborn with Different Gestational Age

摘要(Abstract):
目的:探讨不同胎龄新生儿出生后1 h、7 d及3月时的骨生化代谢标志物及胰岛素样生长因子-1(IGF-1)的变化及趋势。方法:早产儿及足月儿共125例,根据胎龄分为早期早产儿组32例、晚期早产儿组46例及足月儿组47例,分别于生后1 h、7 d及3月时抽取空腹静脉血4 m L,检测血清钙(Ca)、磷(P)、维生素D3(Vit D3)、骨源性碱性磷酸酶(BAP)及IGF-1。结果:3组新生儿血清Vit D3、Ca水平,随胎龄及日龄增加而升高,差异有统计学意义(P<0.05);2组早产儿血清IGF-1水平随胎龄、日龄增加而升高,差异有统计学意义(P<0.05);各年龄段3组新生儿血清BAP水平差异有统计学意义(P<0.05),随日龄增加,晚期早产儿组及足月儿组BAP水平差异有统计学意义(P<0.05);随日龄增加,3组新生儿血清P水平差异有统计学意义(P<0.05);出生后3月时各组新生儿Vit D3、Ca、IGF-1水平达到正常值。结论:早产及足月产新生儿血清Vit D3及血清Ca水平随胎龄及日龄增加而升高,早产儿血清IGF-1水平随出生后日龄增加而升高;出生后3月时早产和足月产新生儿血清Vit D3、Ca及IGF-1水平达到正常值。
Objective: To investigate the change and trend of biochemical markers of bone metabolism and insulin-like growth factor 1( IGF-1) 1 h,7 d and 3 months after neonatal birth. Methods: A total of 125 cases of preterm infants and full-term infants were enrolled in this study. According to different gestational ages,they were divided into early preterm infants group(32 cases),late preterm infants group(46 cases),full-term group(47 cases). For each infant,4 ml fasting blood samples were collected respectively at 1st hour,7th day and 3 months after birth,then the serum levels of calcium(Ca),phosphorus(P),vitamin D3(Vit D3),bone alkaline phosphatase(BAP) and IGF-1 were detected. Results: The serum Vit D3,Ca levels in three groups increased with gestational age and age in days of neonatal infants,the differences were statistically significant( P < 0. 05). The serum IGF-I levels increased with gestational age and age in days of neonatal infants in the two premature groups,the differences weres statistically significant( P < 0. 05). In the three groups of neonatal infants,the serum BAP level's differences in all ages were statistically significant( P < 0. 05). With the increasing of age in days,the differences of BAP levels between late preterm infants and full term group were statistically significant( P < 0. 05). With the increasing of age in days,neonatal serum P level's differences between three groups were statistically significant( P < 0. 05). Vit D3,Ca and IGF-I levels of preterm infants and full-term infants returned to normal 3 months after birth. Conclusion: The serum Vit D3,Ca levels increase with gestational age and age in days of neonatal infants. For preterm infants,the serum IGF-I levels increase with gestational age and age in days of neonatal infants. Vit D3,Ca and IGF-I levels of preterm infants and full-term infants returned to normal 3 months after birth.
