NF-κB表达在胰腺癌的诊断和预后中的价值Diagnostic and Prognostic Value of NF-κB Expression Level in Pancreatic Cancer

摘要(Abstract):
目的:探转录因子B(NF-κB)在胰腺癌组织中的表达水平及与肿瘤转移复发的关系。方法:选择手术及术后随访≥24个月、保存完好的胰腺癌样本67例、相应癌旁组织67例以及同期良性胰腺疾病手术样本45例,采用免疫组织化学方法检测3种组织标本中NF-κB表达水平,制作受试者工作特征(ROC)曲线、计算截断值,观察NF-κB水平对胰腺癌诊断的灵敏度、特异度、阳性预测值及符合率;胰腺癌组患者术后随访24月,比较淋巴结及远处转移患者NF-κB表达水平。结果:胰腺癌组织NF-κB表达水平高于癌旁组织、癌旁组织高于良性胰腺疾病组织,差异有统计学意义(P <0. 05); NF-κB诊断胰腺癌ROC下曲线面积为0. 841,灵敏度、特异度、阳性预测值、符合率分别为91. 0%、75. 6%、84. 7%以及84. 8%;胰腺癌淋巴结转移患者的NF-κB表达水平高于无转移患者(P <0. 05),术后随访24月时肿瘤扩散、转移、复发患者NF-κB表达水平高于无瘤生存的患者(P <0. 05)。结论:NF-κB高表达与胰腺癌复发、转移密切关联。
Objective: To explore the expression level of NF-κB transcription factor B in pancreatic cancer tissue and its association with tumor metastasis and recurrence. Methods: Three types of samples including 67 well-preserved pancreatic cancer samples were collected,their corresponding paracancerous tissue samples from patients with postoperative follow-up for ≥24 months and 45 benign pancreatic disease surgical samples during the same period. Immunohistochemical( IHC) methods were used to detect NF-κB expression levels for generating receiver operating characteristic( ROC)curve. Cut-off value was calculated. The diagnostic value of NF-κB level in pancreatic cancer were evaluated by observing the sensitivity,specificity,positive predictive value and coincidence rate. The patients were followed up for 24 months after surgery. The expression levels of NF-κB were compared between patients with lymph node and with distant metastasis. Results: The expression level of NF-κB was higher in pancreatic cancer tissue than those in adjacent tissues and benign pancreatic disease tissues( P < 0. 05). The area of curve( AUC) under ROC for NF-κB diagnosis of pancreatic cancer was 0. 841. The sensitivity,the specificity,positive predictive value and coincidence rate were91. 0%,75. 6%,84. 7%,and 84. 8%,respectively. The expression level of NF-κB was higher in patients with lymph node metastasis of pancreatic cancer than that in patients without metastasis( P <0. 05). At 24 months after surgery,NF-κB expression level was higher in patients with metastasis and recurrence than those in tumor-free patients( P < 0. 05). Conclusion: The high expression of NF-κB is closely related to the recurrence and metastasis of pancreatic cancer.
