西格列汀与二甲双胍对新诊断2型糖尿病患者胰岛β细胞功能的影响

摘要(Abstract):

目的:探讨西格列汀与二甲双胍对新诊断2型糖尿病患者胰岛β细胞功能的影响。方法:100例新诊断2型糖尿病患者,根据治疗方法分为西格列汀组与二甲双胍组,2组患者均给予相应药物治疗6个月;于治疗前及治疗6个月时,检测2组患者空腹血糖(FPG)、餐后2 h血糖(2hPG)、糖基化血红蛋白(Hb A1c)、总胆固醇(TC)、甘油三酯(TG)、胰岛素抵抗指数(HOMA-IR)及胰岛β细胞功能指数(HOMA-β)水平。结果:治疗前,2组患者FPG、2hPG、Hb A1c、TC、TG、HOMA-IR及HOMA-β水平比较,差异无统计学意义(P> 0. 05);治疗6个月时,2组患者FPG、2hPG、Hb A1c、TC、TG及HOMA-IR水平均较治疗前降低,HOMA-β水平较治疗前升高,差异有统计学意义(P <0. 05);西格列汀组FPG、2hPG及Hb A1c水平低于二甲双胍组,HOMA-β水平显著高于二甲双胍组,差异有统计学意义(P <0. 05); 2组患者TC、TG及HOMA-IR水平比较,差异无统计学意义(P> 0. 05)。结论:与二甲双胍比较,西格列汀治疗能更有效改善新诊断2型糖尿病患者胰岛β细胞的功能。
Objective: To investigate the effects of sitagliptin and metformin on islet β cell function in newly diagnosed type 2 diabetes patients. Methods: Based on treatment, one hundred newly diagnosed type 2 diabetes patients were divided into sitagliptin group and metformin group. The patients were treated with corresponding drugs for 6 months. Before and after the treatment,. fasting blood glucose( FPG),2-hour blood glucose( 2 hPG),glycosylated hemoglobin( HbA1 c),total cholesterol( TC),triglycerides( TG),insulin resistance index( HOMA-IR),and functional index( HOMA-β) level of islet β cells were measured. Results: Before treatment,the levels of FPG,2 hPG,Hb A1 c,TC,TG,HOMA-IR,and HOMA-β in the two groups were not significantly different( P > 0. 05). At 6 months of the treatment,the FPG,2 h PG,HbA1 c,TC,TG,and HOMA-IR levels were significantly lower than before treatment,while HOMA-β levels were significantly higher than those of before the treatment( P < 0. 05). FPG,2 hPG HbA1 c levels were significantly lower in the sitagliptin group than those in the metformin group,while HOMA-β levels were significantly higher than those in metformin group( P < 0. 05). There were no significant differences in TC,TG,and HOMA-IR levels between the two groups( P > 0. 05). Conclusion: Sitagliptin treatment can more effectively improve the function of islet β cells in newly diagnosed type 2 diabetes patients than metformin did.
