支气管哮喘患儿粉尘螨滴剂治疗前后EOS及ECP水平
Levels of EOS and ECP before and after Specific Immunotherapy for Children with Bronchial Asthma

摘要(Abstract):

目的:观察粉尘螨滴剂特异性免疫治疗对支气管哮喘患儿嗜酸性粒细胞(EOS)及嗜酸细胞阳离子蛋白(ECP)的影响。方法:粉尘螨过敏轻中度支气管哮喘患儿60例,均分成常规治疗组(常规组)和(常规加特异性免疫)治疗组(免疫治疗组),常规组仅予吸入糖皮质激素治疗,免疫治疗组吸入糖皮质激素的同时加用粉尘螨滴剂舌下特异性免疫治疗;检测治疗前和治疗后6月、12月EOS及血清ECP水平。结果:(1)两组患儿EOS治疗前差异无统计学意义(P>0.05);治疗12个月,治疗组EOS低于常规组,P<0.05;组内比较,治疗12个月与治疗前比较,常规组P<0.05,免疫治疗组P<0.01,与治疗6月比较,治疗组P<0.05;(2)两组患儿ECP治疗前差异无统计学意义(P>0.05);治疗6个月和12个月,免疫治疗组ECP低于常规组,P<0.05及P<0.01;组内比较,治疗12个月与治疗前比较,常规组P<0.05,免疫治疗组P<0.01,与治疗6月比较,治疗组P<0.01。结论:特异性免疫治疗能降低支气管哮喘患儿EOS及血清ECP水平,且ECP下降更明显,这可能是粉尘螨滴剂特异性免疫治疗对支气管哮喘治疗的有效机制。
Objective: To investigate the effect of dermatophagoides farinae drops immunotherapy on eosinophils(EOS) and the ecidophil cationic protein(ECP) children with asthma. Methods: 60 mild or moderate dust mite sensitivity children with asthma were divided into control group( conventional group) and treatment group(immunotherapy group). The control group only inhaled corticosteroids for therapy,treatment group inhaled corticosteroids combined with dermatophagoides farinae drops therapy. The levels of EOS and ECP were detected before and after 6 and 12 months of treatment. Results:(1) Comparison of same time EOS level between two groups exhibited no statistical significance before treatment(P > 0. 05); EOS decreased obviously after 12 months of treatment(P < 0. 05); comparison within groups before and after 12 months treatment,control group(P < 0. 05) and treatment group(P< 0. 01),comparing with data of after 6 months treatment,treatment group( P < 0. 05);(2) Comparison of same time ECP level between two groups exhibited no statistical significance before treatment(P >0.05); ECP decreased obviously after 6 and 12 months of treatment(P <0.05)(P <0.01);comparison within groups before and after 12 months treatment,control group( P < 0. 05) and treatment group(P < 0. 01),comparing with data after 6 months treatment,treatment group(P < 0. 01).Conclusions: Specific immune therapy can effectively decrease the patient's serum EOS and ECP levels,and ECP drop more obvious. This could suggest the effective mechanism of dermatophagoides farinae drops immunotherapy on bronchial asthma.
