七氟烷复合丙泊酚麻醉用于小儿短小手术的效果Effect of Sevoflurane Combined with Propofol Anesthesia in Pediatric Short Surgery

摘要(Abstract):
目的:探讨七氟烷复合丙泊酚麻醉在小儿短小手术中的应用效果。方法:将40例择期行短小手术患儿均分为七氟烷组(S组)和七氟烷复合丙泊酚组(SP组),七氟烷吸入后常规静脉诱导,行气管内插管机械通气,SP组丙泊酚6 mg/(kg·h)持续静脉泵注复合七氟烷吸入维持麻醉,S组仅七氟烷吸入维持麻醉;记录两组手术患儿诱导前(T0)、手术开始(T1)、手术结束(T2)、拔除气管导管时(T3)及拔管后5 min(T4)时心率(HR)、平均动脉压(MAP)、脉搏氧饱和度(Sp O2)变化,比较两组术后拔管时间、苏醒时间、术中七氟烷用量和苏醒期并发症。结果:各时点两组患儿HR、MAP、Sp O2、拔管时间、苏醒时间比较无差异(P>0.05);与S组比较,SP组七氟烷药量减少、术后躁动降低(P<0.05)。结论:小儿短小手术应用七氟烷复合丙泊酚麻醉,血流动力学稳定,术后并发症少,效果良好。
Objective: To observe the effect of sevoflurane combined with propofol anesthesia in pediatric short surgery. Methods: Forty children undergoing elective short surgery were randomly divided into two groups( n = 20 each) : sevoflurane group( S) and sevoflurane combined with propofol group( SP). Two groups of children received midazolam 0. 1 mg /kg,atropine 0. 01 mg /kg,fentanyl 3 μg /kg and dexamethasone 0. 1 mg / kg after they inhalated sevoflurane,then endotracheal intubation and mechanical ventilation were conducted. Group S only received sevoflurane inhalation to maintain anesthesia while group SP received propofol 6 mg( kg·h) continuous intravenous injection combined with sevoflurane inhalation. HR,MAP and Sp O2 were recorded before the induction of anesthesia( T0),the start of surgery( T1),the end of surgery( T2),during the extubation( T3) and 5 minutes after extubation( T4). The extubation time,awakening time and the complication during awakening were compared between the two groups. Results: The differences of HR,MAP and Sp O2 at T0,T1,T2,T3,T4,extubation time and awakening time between group S and group SP were not statistically significant in two groups( P > 0. 05). Compared with group S,the total dose of sevoflurane was lower in group SP and the postoperative agitation in group SP was lower,and the differences were statistically significant( P < 0. 05). Conclusion: Sevoflurane combined with propofol anesthesia in pediatric short surgery can make the hemodynamics stable,cause fewer complications and obtain better effect.
