子宫内膜癌患者外周血调节性T细胞与肿瘤免疫的相关性

摘要(Abstract):

目的:研究子宫内膜癌(EC)患者外周血CD4+CD25+CD127LOW和CD4+CD25+foxp3+调节性T细胞(Treg)的百分比(CD4+CD25+CD127LOWTreg/CD4、CD4+CD25+Foxp3+Treg/CD4)与EC患者血清白细胞介素-10(IL-10)、转化生长因子-β1(TGF-β1)、TGF-β2水平的相关性,探讨CD4+CD25+Treg在肿瘤免疫逃逸中的作用机制。方法:30例确诊的EC患者(EC组)及30例健康体检女性(对照组),用流式细胞术分别检测EC组手术前后和化疗后及对照组外周血CD4+CD25+CD127LOWTreg/CD4、CD4+CD25+Foxp3+Treg/CD4,同时用ELISA法检测上述时点2组被检者血清IL-10、TGF-β1及TGF-β2含量;比较对照组、EC组手术前外周血CD4+CD25+CD127LOWTreg/CD4、CD4+CD25+Foxp3+Treg/CD4及血清TGF-β1、TGF-β2、IL-10含量,比较EC患者手术前、手术后及化疗后外周血CD4+CD25+CD127LOWTreg/CD4、CD4+CD25+Foxp3+Treg/CD4及血清TGF-β1、TGF-β2及IL-10含量,采用spearman相关分析法分析EC患者外周血CD4+CD25+CD127LOWTreg/CD4、CD4+CD25+Foxp3+Treg/CD4与手术前血清IL-10、TGF-β1及TGF-β2含量的相关性。结果:与对照组比较,EC组患者手术前外周血CD4+CD25+CD127LOWTreg/CD4、CD4+CD25+Foxp3+Treg/CD4、血清IL-10的含量均明显升高,TGF-β2含量明显降低(P<0.01);与手术前比较,EC组患者手术后及化疗后CD4+CD25+CD127LOWTreg/CD4、CD4+CD25+Foxp3+Treg/CD4、血清IL-10和TGF-β2含量明显降低(P<0.01),手术后血清TGF-β1含量无明显变化(P>0.05),而化疗后血清TGF-β1含量明显降低(P<0.01);EC组患者CD4+CD25+CD127LOWTreg/CD4与血清TGF-β1含量存在正相关(r=0.284,P=0.049),CD4+CD25+Foxp3+Treg/CD4与血清IL-10含量存在正相关(r=0.287,P=0.047)。结论:EC患者外周血CD4+CD25+CD127LOWTreg、CD4+CD25+Foxp3+Treg水平上调,可能是肿瘤产生免疫耐受,发生肿瘤免疫逃逸的原因。
Objective: To investigate the relationship between two types of regulatory T cells( CD4+CD25+CD127LOWTreg and CD4+CD25+Foxp3+Treg) expression in peripheral blood of patients with endometrial cancer( EC) and tumor immunity. Methods: 35 patients with EC were selected and 30 healthy female as healthy controls,flow cytometry was used to detect the proportion of CD4+CD25+CD127LOWTreg and CD4+CD25+foxp3+Treg accounted for CD4+T lymphocyte expression in peripheral blood in EC group and healthy controls before and after surgery and chemotherapy; ELASA was used to detect content of IL-10 cytokines,TGF-β1and TGF-β2in serum with the same specimens. Results:The proportion of CD4+CD25+CD127LOWTreg and CD4+CD25+foxp3+Treg accounted for CD4+T lymphocyte in peripheral blood in patients and content of IL-10 in serum with EC group were significantly higher than that in healthy controls( P < 0. 01); the content of TGF-β1 in serum was no statistically significant different in two groups( P > 0. 05); The proportion of CD4+CD25+CD127LOWTreg and CD4+CD25+foxp3+Treg accounted for CD4+T lymphocyte in peripheral blood in patients and content of IL-10 and TGF-β2in serum with endometrial cancer group were significantly decreased after surgery and chemotherapy( P < 0. 01); there was no statistically significant difference in the content of TGF-β1in serum after surgery( P > 0. 05); the content of TGF-β1in serum were significantly decreased after chemotherapy( P < 0. 01). There was positive correlation between CD4+CD25+CD127LOWTreg and content of TGF-β1( r = 0. 284,P = 0. 049) with EC group; there was positive correlation between CD4+CD25+foxp3+Treg accounted for CD4+T lymphocyte and the content of IL-10( r= 0. 287,P = 0. 047). Conclusion: The levels of regulatory T cells increased obviously in peripheral blood in patients with EC,this may be the reason for tumor immunological tolerance and tumor immune escape.
