摘要(Abstract)：

目的:探讨影响胰岛素泵治疗后1型糖尿病(T1DM)患者血糖达标的因素。方法:采用胰岛素泵治疗的T1DM成人患者100例,根据治疗后糖化血红蛋白(Hb A1c)水平分成达标组(Hb A1c≤7.0%)和未达标组(Hb A1c>7.0%),比较两组患者年龄、病程、体重指数(BMI)、性别比、餐后2h C肽水平、推荐饮食的执行人数、每周不同运动时间人数、血糖监测频次、更换胰岛素输注管路频率、不同受教育程度患者的比例、不同人均年收入家庭比例、胰岛素泵每日基础量、每日总量、基础量/总量比值、每日餐前胰岛素剂量、基础率分段、患者低血糖频率、使用不同类型胰岛素比例及不同慢性并发症患者比例;采用多元回归分析不同Hb A1c含量与影响因素的相关性。结果:达标组患者的胰岛素泵每日基础量、胰岛素泵基础量/胰岛素泵全天总量明显低于未达标组,而血糖监测频次高于未达标组,差异有统计学意义(P<0.05),其余指标比较差异均无统计学意义(P>0.05);多元线性回归分析显示,胰岛素泵治疗的T1DM成人患者血糖达标与性别、血糖监测频次成正相关(β=0.339、0.291,P<0.05),与每日胰岛素泵基础量负相关(β=-1.547,P<0.05)。结论:性别、血糖监测频次与胰岛素每日基础量是胰岛素泵治疗T1DM成人患者血糖达标的影响因素。
Objective: To explore the influencing factors of blood glucose control on T1 DM patients with insulin pump therapy. Methods: 100 grown-up patients with T1 DM in were divided into 2 groups(Group A: Hb A1c≤7%,Group B: Hb A1 c > 7. 0%). Clinical data and self-management were collected and analyzed between two groups: age,course of diseases,BMI,empty belly C peptide,2 h postprandial c-peptide,adhere to the recommended number of diet,exercise time each week,replacement of insulin infusion line frequency,level of education,family income per head,high-dose daily before a meal,segmentation base rate,low frequency,the types of insulin,chronic complications. Adopting Multivariate Regression Analysis to analyze different Hb A1 c content and influencing factors relevance. Results: daily insulin pump,basic insulin pump / whole day content of standard group was obviously lower than substandard group; blood glucose monitoring frequency was higher than substandard group,difference was statistically significant(P < 0. 05); other indexes comparison showed no statistical significane(P > 0. 05). Multivariate Regression Analysis showed that insulin pump treatment on T1 DM adult patients was positive correlated with gender,blood glucose monitoring frequency,while negative correlated wih daily insulin pump basic content(P < 0. 05). Conclusion: Gender,blood glucose monitoring frequency and insulin daily basis are influencing factors on T1 DM patients with insulin pump therapy.
