经颅磁刺激治疗对重性抑郁障碍患者血清BDNF和5-HT水平的影响
The Impact of Repetitive Transcranial Magnetic Stimulation on BDNF and 5-HT Expression of Major Depressive Disorder Patients

摘要(Abstract):

目的:探讨经颅磁刺激治疗对重性抑郁障碍(MDD)患者血清脑源性神经营养因子(BDNF)及5-羟色胺(5-HT)水平的影响。方法:120例MDD患者均分为观察组和对照组,对照组给予帕罗西汀20 mg/d口服治疗6周,观察组在对照组治疗的基础上同时给予经颅磁刺激治疗2周,比较2组患者治疗1、2、3及6周时抑郁缓解率,治疗前和治疗6周时的蒙哥马利抑郁量表(MADRS)、汉密尔顿抑郁量表(HAMD)及简易精神状态检查量表(MMSE)评分;采用酶联免疫吸附法测定两组患者治疗前和治疗6周时血清BDNF、5-HT水平。结果:治疗1、2、3及6周时,观察组患者抑郁缓解率显著高于对照组(P<0.05);治疗前,两组患者MADRS、HAMD、MMSE评分及血清BDNF、5-HT水平比较,差异无统计学意义(P>0.05),治疗6周时,两组患者MADRS、HAMD评分低于治疗前,血清BDNF、5-HT水平高于治疗前(P<0.01);治疗6周时,观察组MADRS、HAMD及MMSE评分明显低于对照组,血清BDNF、5-HT水平高于对照组(P<0.01)。结论:重复经颅磁刺激联合帕罗西汀能有效改善MDD患者躯体症状及认知功能,能快速有效改善患者抑郁状态,可能机制为重复经颅磁刺激能有效促进大脑组织释放BDNF、5-HT有关。
Objective: To investigate the effect,of repetitive transcranial magnetic stimulation treatment on the levels of serum brain-derived neurotrophic factor( BDNF) and serotonin( 5-HT) in patients with major depressive disorder( MDD). Methods: 120 cases of MDD patients were divided into observation groups( n = 60) and control group( n = 60). The control group was given paroxetine20 mg / d,observation group received transcranial magnetic stimulation for two weeks. Both groups were treated for 6 weeks. The depression remission rate of two groups after 1 week,2 weeks,3 weeks and 6 weeks were recorded. The depression and cognitive function evaluation of two groups were evaluated by Montgomery Depression Rating Scale( MADRS),the Hamilton Depression Scale( HAMD)and the Mini-Mental State Examination( MMSE). The levels of BDNF,5-HT of two groups were tested by ELISA. Results: The depression remission rate of observation group after treatment for one week,two weeks,three weeks and six weeks,was significantly higher than control group( P < 0. 05).The levels of MADRS score,HAMD score,MMSE scores of observation group were lower than the con-trol group( P < 0. 05). The levels of BDNF,5-HT of observation group after treatment were higher than control group( P < 0. 01). Conclusion: Repetitive transcranial magnetic stimulation combined with paroxetine MDD can effectively improve the physical symptoms and cognitive function in patients with MDD,it can quickly and effectively improve the patient depression,it is possible that repetitive transcranial magnetic stimulation can effectively promote BDNF and 5-HT release.
