西地那非联合贝前列素钠治疗先天性心脏病相关性肺动脉高压
Clinical Study on Sildenafil and Bainprost Sodium in the Treatment of Pulmonary Arterial Hypertension Associated with Congenital Heart Disease

摘要(Abstract):

目的:观察西地那非联合贝前列素钠治疗先天性心脏病相关性肺动脉高压(PAH-CHD)的临床效果。方法:43例PAH-CHD患者随机分为观察组(n=20,口服西地那非联合贝前列素钠)和对照组(n=23,口服西地那非),分别于服药前和服药后第1、3、6及12月测定2组患者肺动脉收缩压、6 min步行试验(6MWT)和心功能分级(NYHA)。结果:两组PAH-CHD患者治疗6月及12时肺动脉收缩压均降低,与同组治疗前比较差异有统计学意义(P<0.05),但同组6月与12月肺动脉收缩压比较差异无统计学意义(P>0.05),治疗12月时观察组肺动脉收缩压降低更明显(P<0.05);6MWT结果显示,与服药前比较,观察组于服药后第1个月、对照组于服药后第3个月开始,6MWT步行距离显著增加(P<0.05);第6个月时观察组6MWT步行距离长于对照组(P<0.05);与服药前比较,观察组服药后第3个月时开始,对照组第6个月开始NYHA分级的Ⅲ级和Ⅳ级患者比例明显减低,Ⅰ级和Ⅱ级患者比例明显升高(P<0.05),同一治疗时点两组Ⅰ级和Ⅱ级患者比例比较,差异无统计学意义(P>0.05)。结论:西地那非联合贝前列素钠治疗PAH-CHD可有效降低肺动脉收缩压,改善心功能。
Objective: To investigate the curative efficacy of sildenafil and bainprost sodium in the treatment of pulmonary arterial hypertension associated with congenital heart disease( PAH-CHDP).Methods: 43 patients with PAH-CHD were divided into the observation group( n = 20) and the control group( n = 23). The former took oral sildenafil and bainprost sodium,while the latter took oral sildenafil only. Pulmonary artery systolic pressure,six minute walk test( 6 MWT) and New York Heart Association( NYHA) Classification of both groups were detected before and in the 1~(st),3~(rd),6~(th) and 12~(th) months after treatment. Results: The pulmonary artery systolic pressure declined( P < 0. 05) in the 6~(th) and 12~(th) months,and there was statistical significance compared with the index before the treatment,while the difference between the declined index in the 6 th month and the 12~(th) month after treatment had statistical significance( P > 0. 05). The pulmonary artery systolic pressure declined more obviously in the 12~(th) month after treatment. 6 MWT showed that the distance began to increase obviously in the 1~(st) month in the observation group and in the 3~(rd) month in the controls after taking the drug( P <0. 05),but the comparison of the index in the 3~(rd),6~(th) and 12~(th) months had no statistical significance( P > 0. 05); The heart function in the 3~(rd) month after taking the drug began to improve( P < 0. 05),no significant difference in the 3~(rd) and 6~(th) months( P > 0. 05),yet there was significant difference between the 6~(th) and 12~(th) months after treatment( P < 0. 05); in the control group,PAH began to decline in the 6~(th) month after taking the drug( P < 0. 05),no significant difference between the indexes in the 12~(th) and 6~(th) months. Compared with the data before treatment,the proportion of NYHA Class Ⅲand Ⅳ began to decline in the observation group from the 6~(th) month and in the control group from the 3~(rd) month,while the proportion of Class Ⅰ and Ⅱ increased( P > 0. 05). There was no significant difference between the proportions of Class I and Ⅱ in both groups during the same period of treatment. The curative efficacy of both groups showed no significant difference in cardiac function. Conclusions: Oral sildenafil combined with sildenafil and beraprost sodium can be applied to the treatment of PAHCHD,which is safe and reliable,and it can effectively reduce PAH,increase 6 MWT distance and improve cardiac function.
