胸水HPV16、HIF1α及VEGF检测对小细胞肺癌的诊断价值

摘要(Abstract):

目的:探讨胸水人乳头瘤病毒16型(HPV16)、缺氧诱导因子1α(HIF1α)及血管内皮生长因子(VEGF)检测对小细胞肺癌的诊断价值。方法:选取79例小细胞肺癌伴胸水患者作为观察组、同期68例良性胸水患者为对照组,应用酶联免疫吸附法检测两组患者胸水中HPV16、HIF1α及VEGF水平,分析3项指标单独检测或联合检测在小细胞肺癌诊断及分期中的应用价值。结果:胸水HPV16、HIF1α、VEGF联合检测诊断小细胞肺癌的灵敏度及特异度均高于3项指标单独检测,差异有统计学意义(P<0.05);胸水HPV16、HIF1α及VEGF联合检测在小细胞肺癌Ⅰ~Ⅱ期、Ⅲ~Ⅳ期中阳性表达率及总表达率均高于3项指标单独检测,差异有统计学意义(P<0.05)。结论:胸水HPV16、HIF1α、及VEGF联合检测在小细胞肺癌诊断中灵敏度及特异度高,可为小细胞肺癌早期诊断提供依据。
Objective: To investigate the expression of human papillomavirus type 16( HPV16),hypoxia inducible factor 1α( HIF1α) and vascular endothelial growth factor( VEGF) in pleural effusions of small cell lung cancer and its differential diagnostic value. Methods: 79 cases with small cell lung cancer combined with pleural effusion treated in our hospital from February 2013 to May 2017 were selected as the observation group,while 68 patients with benign pleural effusion were selected as the control group. The levels of HPV16,HIF1α and VEGF in the two groups were detected by the enzymelinked immunosorbent assay( ELISA) to access the clinical value of the three indexes in the differential diagnosis of small cell lung cancer. Results: The specificity of combined detection of HPV16,HIF1α and VEGF was higher than that of HPV16,HIF1α and VEGF,respectively( P < 0. 05). The positive rates of HPV16,HIF1α and VEGF in Stages Ⅰ ~ Ⅱ and Ⅲ ~ Ⅳ of SCLC were higher than those in the three separate tests( P < 0. 05). Conclusions: HPV16,HIF1αand VEGF are all expressed in pleural effusions of small cell lung cancer. The joint detection of the three indicators has high sensitivity and specificities in the diagnosis of small cell lung cancer. It provides a scientific and comprehensive theoretical basis for the early diagnosis of small cell lung cancer for further improvement of the quality of prognosis rehabilitation.
